摘 要:目的探讨多层螺旋CT横断面及多平面重建在恶性肿瘤侵犯颈部血管中的临床应用价值。方法选择我院2002年10月至2012年3月经手术及病理证实的41例颈部恶性肿瘤患者作为研究对象,回顾性分析患者的多层螺旋CT横断面及多平面重建图像资料,总结受侵血管与未受侵血管的图像情况。结果横断面扫描图像显示,肿瘤包绕颈部血管的角度在90°~180°以及>180°对颈部血管受侵判断的准确度均明显高于<90°组,差异有统计学意义(P<0.05),综合敏感度、特异度考虑以90°~180°组对颈部血管受侵诊断价值最高;多平面重建图像显示,肿瘤与颈部血管交界面角度≥90°时受侵血管率(10/18)明显多于角度<90°时(4/30),差异有统计学意义(P<0.05)。结论多层螺旋CT横断面及多平面重建对颈部恶性肿瘤侵犯颈部血管有重要的诊断价值,横断面扫描图像肿瘤包绕颈部血管的角度在90°~180°、多平面重建图像肿瘤与颈部血管交界面角度≥90°有助于作出颈部血管受侵诊断。Objective To explore the clinical application value of multi-slice spiral CT cross section and multi-planar reconstruction on the involvement of cervical vessels by neck malignant tumor. Methods From October 2002 to March 2012 in our hospital,41 cases of neck malignant tumor proved by operation and pathology were selected, their image data of multi-slice spiral CT cross section and multi-planar reconstruction were analyzed retrospectively, the image situation Of invaded vessels and without invaded vessels were summarized. Results The image of cross section showed that the tumor enveloping cervical vessels of 90° to 180° and 〉 180°were significantly higher than 〈 90° group, the differences were statistically significant ( P 〈 0. 05 ) , and the sensitivity and specificity of 90° to 180° group were acceptable; the image of multi-planar reconstruction showed that the invaded vessels were more than without invaded vessels when the tumor and neck vascular interface angle≥ 90°, the difference was statistically significant (P 〈 0. 05 ). Condusions Multi-slice spiral CT cross section and multi-planar reconstruction has important diagnostic value on the involvement of cervical vessels by neck malignant tumor, the tumor enveloping cervical vessels of 90° to 180°in cross section image and the tumor and neck vascular interface angle≥90° in muhi- planar reconstruction image are helpful to the diagnosis of neck vascular invasion.
