栅栏状包膜神经瘤1例

摘　　要：患者男,27岁。下唇边缘皮疹4年余。皮肤科情况:下唇边缘可见一肤色圆顶丘疹,界清,质中,表面光滑,缓慢增大,无自觉症状。皮损组织病理示:真皮内可见数个结节状的肿瘤团块,界清,外有不完整包膜。瘤体内见多数呈轻度波状的神经束,胞核梭形,部分呈栅栏状排列,束间可见裂隙形成及黏液性基质。免疫组化示:团块内细胞S-100弥漫(+),周围包膜EMA(+)。诊断:栅栏状包膜神经瘤。A 27-year-old man presented with an eruption on the lower lip for 4 years. Physical examination revealed that a well-demarcated, smooth skin - colored papule with medium hardness on the edge of the lower lip. The lesion was asymptomatic and gradually increased in size. Histopathological examination of the papule showed that in the dennis, there were several partially encapsulated, well-circumscribed nodules composed of fascicles of small spindle cells with slightly contoured nuclei and foci of palisaded growth, interlaced with prominent clefts and mucoid matrix. Immunohistochemical staining showed that the spindle cells were diffusively stained positive for S100 protein and the capsule stained positive for EMA. Based on the above clinical and histopathological findings, a diagnosis of palisaded encapsulated neuroma was made.
