摘 要:目的:研究载银二氧化钛纳米管种植体表面涂层对周围软组织炎症反应的影响。方法:将纯钛抛光组(PT)、TiO2纳米管组(NT)、低浓度载银TiO2纳米管组(NAg-L)和高浓度载银TiO2纳米管组(NAg-H)(n=12)种植钉沿颊舌向植入成年家犬双侧下颌骨内,术后7、14及28 d提取种植体周围龈沟液(PICF)并取材制作切片,观察软组织内炎症细胞浸润情况;采用实时定量PCR检测周围软组织内TNF-αmRNA的表达量。结果:PICF体积在7 d时NAg-L组最少,NAg-H组最多,分别与PT组比较,P<0.05;14 d时NAg-H组降低;28 d时4组间无统计学差异。组织学观察发现NT组及NAg组相对于PT组皆具有较少的炎细胞浸润。TNF-αmRNA的表达量7 d时NAg-L组明显低于其它3组;14 d时NAg-H组最低,且各组间两两比较皆具有统计学差异;28 d时4组间无统计学差异。结论:种植体表面加载低剂量纳米银颗粒可以有效抑制术区炎症反应程度。Objective: To investigate the influence of the modified surface of titanium dental implant on inflammatory infiltration in pe-ri-implant soft tissue. Methods: The titanium dental implants were divided into 4 groups( n = 12) 1 y different surface treatment: Polished titanium(PT), Tin2 nanotube surface(NT) , Ag-loaded Tin2 nanotube with low density(NAg-L) and Ag-loaded Tin2 nanotube with high density(NAg-H). The implants were inserted in each side of the mandible of the dogs using flapless technique. 7, 14 and 28 d after surgery, peri-implant crevicular fluid (PICF) were measured and mucosa biopsies were processed for histological observation to verify the soft tissue inflammatory infiltration; TNF-α gene expression was examined by RT-PCR. Results: The minimum PICF volume on day 7 was found in NAg-L group, but on day 14 PICF in NAg-H group decreased significantly. On day 28 there was no significant difference among 4 groups. Lower inflammatory infiltration was observed in NT group and Ag-loaded nanotube groups than in PT group. TNF-ct mRNA expression on day 7 in NAg-L group was the lowest;but on day 14 and 28 in NAg-H group was the lowest; between PT group and NAg-H group, P 〈 0.05. Conclusion: Ag-loaded with low dose on implant TiO2 nanotube surface may decrease inflammation infiltration in peri-implant soft tissue.
