摘 要:目的应用脉冲多普勒测量心室Tei指数,探讨母体高血压环境对胎儿心功能的影响及其出生后的心功能状况。方法选择孕28—41“周的胎儿185例,产后1~7d的新生儿203例,产后4—5个月婴儿174例。胎儿、新生儿及婴儿均设对照组和研究组,研究组分为妊娠期高血压(GH)组、轻度子痫前期(MPE)组、重度子痫前期(SPE)组。对所有研究对象进行左右心室Tei指数的测量并进行比较。结果GH组与对照组胎儿左右心室Tei指数比较差异无统计学意义;MPE组、SPE组胎儿左右心室Tei指数大于对照组胎儿(P均〈0.05)。GH组、MPE组、SPE组足月产与对照组足月产新生儿比较,左右心室Tei指数无显著性差异。对照组早产与对照组足月产新生儿左右心室Tei指数比较无显著性差异。SPE组早产、MPE组早产新生儿左右心室Tei指数均大于对照组足月产新生儿(P均〈0.05)。GH组、MPE组、SPE组足月产4~5个月婴儿与对照组足月产4~5个月婴儿比较,左右心室Tei指数无显著性差异。SPE组、MPE组及对照组早产4~5个月婴儿与对照组足月产4~5个月婴儿比较,左右心室Tei指数无显著性差异。结论子痫前期胎儿左、右心室整体功能均受损害;单纯的早产因素并不引起新生儿心功能的改变;子痫前期胎儿在生后4~5个月内Tei指数均恢复正常水平。Objective It is to explore the influence of maternal hypertension environment on cardiac function of fetus and neonate evaluated with ventricular Tei index by pulsed Doppler. Methods 185 cases of 28 weeks to 41 weeks and 6 days old fetuses,203 cases of 1 -7 days old neonates and 174 cases of 4 -5 months old infants were selected. Fetus, neonatal and in- fants were divided into control group and study group. The study groups were divided into gestational hypertension (GH) group, mild preeclampsia (MPE) group and severe preeelampsia (SPE) group. The left and right ventricle Tei index of each case was examined and compared. Results Compare the left and right ventricular Tei index of GH group with the control group, there was no statistically significant difference(P 〉 0.05) , but the indexes in MPE group and SPE group were higher than that of the control group (P 〈 0.05 ). Compare the left and right ventricular Tei index of full-term neonate in GH group, MPE group, SPE group with control group, there were no statistical significance (P 〈 0.05). Compare the left and right ventricular Tei index of preterm neonate group with full-term neonates in control group, the differences was not statistically significant ( P〉0. 05 ). Left and right ventricular Tei indexes of premature neonatal in SPE group and MPE group were greater than full-term neonate in control group, the differences was statistical significant ( P 〈 0.05 ). Compare left and right ventrieular Tei index of 4 - 5 months old full-term infant in GH group, MPE group, SPE group with 4 - 5 months old full-term infant in control group, there was no statistical significant difference ( P 〉0. 05 ). The differences of left and right ventricular Tel index between 4 - 5 months old preterm infants in SPE group, MPE group and 4 - 5 months old full-term infants in control groups were not statistical significant (P 〉 0. 05 ).
