TACE联合超声介入治疗门静脉癌栓的临床应用研究

摘　　要：目的比较TACE联合超声介入门静脉插管化疗与无水酒精注射治疗门静脉癌栓的疗效。方法 42例原发性肝细胞肝癌合并门静脉癌栓患者随机分为TACE联合门静脉插管化疗组22例、TACE联合门静脉内癌栓无水酒精注射组20例,两组在病情、病程、病变程度、分类、年龄、性别、肿瘤大小和肿块数等方面无显著差异。观察门静脉癌栓变化。结果 TACE联合门静脉插管化疗组19例门静脉癌栓缩小或消失(86.4%),联合门静脉癌栓内无水酒精注射组16例门静脉癌栓缩小或消失(80.0%);两组治疗癌栓有效率差异无显著性(P>0.05)。结论 TACE联合门静脉插管化疗与TACE联合无水酒精注射治疗原发性肝细胞肝癌合并门脉癌栓均能明显使门脉癌栓缩小或消失,但其差异无显著性。【Objective】 To compare the effects of TACE combined chemotherapy percutaneous portal vein catheterization and percutaneous ethanol injection treatment of portal vein tumor thrombus. 【Methods】42 patients with primary hepatocellular carcinoma with portal vein thrombosis were randomly divided into TACE group with portal vein infusion chemotherapy（22 patients）, TACE in combination with portal vein tumor thrombus alcohol injection（20 patients）. The illness, disease duration, disease degree, classification, age, gender, tumor size and tumor number for two groups were no significant difference.The changes in portal vein tumor thrombuswere observed. 【Results】 Portal vein infusion chemotherapy TACE 19 cases of portal vein tumor thrombus reduced or disappeared（86.4%）, in combination with portal vein tumor thrombus in 16 cases of percutaneous ethanol injection group reduction or disappearance of portal vein tumor thrombus（80.0%）; two groups had tumor thrombus no significant difference of efficiency（P ＆gt;0.05）.【Conclusion】TACE combined chemotherapy Percutaneous portal vein catheterization and TACE combined with percutaneous ethanol injection and the treatment of primary hepatocellular carcinoma with portal vein tumor thrombus to portal vein tumor thrombus could significantly shrink or disappear, but the difference was not statistically significant.
