早期肠内营养支持治疗重症脑卒中患者35例

摘　　要：目的观察早期肠内与肠外营养支持在重症脑卒中患者中的应用效果。方法随机选取医院伴有吞咽困难的重症脑卒中患者70例,均分为两组,观察组接受肠内营养支持,对照组接受肠外营养支持。观察并统计两组患者的平均住院时间、住院费用、并发症发生情况,以及治疗后血清蛋白(Alb)、丙氨酸氨基转移酶(ALT)和血糖(GLU)水平的变化。结果治疗后,观察组患者的平均住院时间为(4.1±1.4)d,平均住院费用为(1.2±0.3)万元,Alb为(39.1±3.1)g/L,ALT(52.6±6.1)g/L,GLU为(10.5±5.5)g/L,并发症发生率为51.43%;对照组患者的平均住院时间为(6.2±1.6)d,平均住院费用为(1.7±0.4)万元,Alb为(34.8±3.2)g/L,ALT为(65.1±5.9)g/L,GLU为(15.7±4.2)g/L,并发症的发生率为88.57%。两组各项指标对比,差异有统计学意义(P<0.05)。结论早期肠内营养支持在治疗重症脑卒中患者时可发挥更好的作用,有效缩短住院时间,节约医疗费用,同时可降低并发症发生率,使患者的营养指标保持在更好的水平。Objective To observe and compare the application effects of early enteral and parenteral nutrition support in the patients with severe cerebral apoplexy. Methods 70 patients with severe cerebral apoplexy admitted to our hospital were chosen and equally di- vided into the two groups. The observation group was given the enteral nutrition support and the control group received the parenteral nutrition support. The average hospitalization duration, hospitalization expense, occurrence of complications and change of Alb, ALT and GLU after treatment were statistically analyzed and compared between the two groups. Results After treatment, in the observation group, the average hospitalization time was （4. 1 ± 1.4）d, the hospitalization expense was （1.2 ±0. 3） ten thousand Yuan, the value of Alb was （39.1 ±3. 1）g/L, the value of ALT was （52.6 ±6. 1）g/L, the value of GLU was （10. 5±5.5）g/L,and the occurrence rate of compli- cations was 51.42%.In the control group, the average hospitalization time was（6.2±1.6）d, the hospitalization expenses was （1.7 ±0. 4） ten thousand Yuan, thevalue of Alb was （34.8±3.2）g/L, the value of ALT was（65.1 ±5.9）g/L, the value of GLU was （15.7 ±4. 2）g/L and the occurrence rate of complications was 88.57%.The differences in various indexes between the two groups had statistical significance（ P 〈 0. 05）. Conclusion Early enteral nutrition support plays an important role in treating nutrition absence pa- tients with severe cerebral apoplexy, can effectively shorten the hospitalization duration, save the medical costs, at the same time can reduce the occurrence rate of complications and maintain the patient＇s nutritive indexes in better level.
