摘  要:目的初步探讨BAL治疗ALF的方法与效果,为进入临床1期试验提供依据。方法构建包括双泵、肝素化、恒温、微囊化活性炭吸附及肝细胞生物反应器-中空纤维管的BAL循环系统,在循环回路中采用高流量再循环技术。将实验动物随机分成对照组(无肝细胞灌流组)与治疗组(肝细胞灌流组)两组。在细胞准备、模型制作及BAL循环系统安装完成后,对照组及治疗组分别进入实验。通过组间肝脏生化指标、生存时间及肝脏病理改变的差异性分析,判别BAL的体外肝脏支持效能。结果与对照组相比较,治疗组生化指标TBil,Alb,Amm,BUN,Cr,PT,Fib的变化水平差异显著,ALT,AST无差异,生存时间差异显著,肝脏病理改变无差异。结论原代正常猪肝细胞-中空纤维BAL可产生维持生命的多种肝功能,治疗猪ALF是有效的。AIM To probe into the method and effect of the treatment of acute liver failure (ALF) with bioartificial liver and provide basis for phase Ⅰ clinical trial. METHODS The circulating system of bioartificial liver consisted of double pumps, heparinizing, constant-temperature-keeping and micro-capsulized activated carbon-adsorbing devices and hepatocytes reactor-hollow fiber module inoculated with microcarriers-adhering hepatocytes. In the circulation loop, high volume recirculating technique was adopted. The experimental pigs were randomized into two groups: control group (perfused without hepatocytes, n = 6 ) and treatment group (perfused with hepatocytes, n = 6 ). After hepatocytes were prepared, models established and circulating system of BAL fired, the control group and the treatment group entered the experiments separately. The other analysis of liver biochemical indexes, survival time and pathological changes of the livers was used to judge the extracorporeal liver support efficiency of BAL. RESULTS Compared with the control group, the biochemical indexes of the treatment group such as TBil, AIb, Amm, BUN, Cr, PT and Fib changed with obvious difference (P<0.01), ALT and AST changed with no difference (P>0.05), survival was significantly different statistically (P<0.01), but little difference in pathological changes. CONCLUSION The BAL of hollow fiber module inoculated with normal primary porcine hepatocytes could produce a variety of hepatic functions which would meet the needs of life support and was effective in treating ALF.
