胚胎骨引导骨再生膜下植入对颌骨缺损修复的意义

摘　　要：目的　评价胚胎骨 (FetalBone ,FB)的空间维持能力和聚乳酸膜 (PLA)胚胎骨复合植入 (PLA/FB)的骨修复效果。方法　在 18只日本大耳白兔双侧下颌骨下缘形成方形骨缺损 ,一侧缺损表面以PLA膜严密覆盖 ,另一侧缺损内植入FB后表面覆盖PLA膜。术后 4、8、12周分期处死动物 ,标本行大体观察、X线检查及组织学观察。结果　术后各期单纯PLA膜侧多数标本发生程度不同的膜塌陷 ,复合植入侧仅 3例发生轻度膜塌陷。组织学观察 ,术后 4、8周复合植入侧成骨量高 ,骨组织丰富。 12周时复合植入侧再生皮质骨连续致密。结论　PLA/FB复合移植修复骨缺损成骨活性高 ,骨量多 。Objective To evaluate the space maintainging capacity of fetal bone (FB).Methods Bilateral mandibular defects were created in 18 adult rabbits.Left defects were covered by PLA membrane alone.The right defects were filled with fetal bone and then covered by PLA membrane.Rabbits were sacrified at 4、8、12 weeks postoperatively.The specimens were assessed by gross inspection,radiograph and light microscopw.Results Collapsed membrane were abserved over the most left defects.Only three PLA membrane collapsed into the defects among the 18 right specimens.Meanwhile,much more new bone formed in the right defects at 4、8 weeks postoperation.Conclusion Fetal bone could play an important role in maintaining bone regeneration space.
