摘 要:目的:了解高原地区藏族妇女宫颈人乳头瘤病毒(HPV)感染率及基因亚型分布特点。方法:采用医用核酸分子快速导流杂交基因芯片技术,对7051名藏族妇女宫颈脱落细胞标本进行HPV基因分型检测。高危型15种(HPV16、18、31、33、35、39、45、51、52、53、56、58、59、66、68),低危型6种(HPV6、11、42、43、44、CP8304)。结果21种HPV亚型均被检出,HPV总感染率19.50%(1375/7051),HPV16出现的频率最高(32.29%),其次为HPV58(19.64%)、HPV52(10.25%)、HPV53(7.42%)、HPV18(5.96%)。其中高危型、单纯低危型、单一和多重HPV感染率分别为93.60%(1287/1375)、6.40%(88/1375)、88.58%(1218/1375)、11.49%(158/1375)。存在着两个感染高峰年龄,第一个高峰组在<25岁(24.23%),第二个高峰组在≥55岁(30.54%)。结论:高原地区藏族妇女宫颈常见亚型是HPV16、HPV58、HPV52、HPV53、HPV18,以高危型和单一HPV感染为主,单纯低危型和多重感染率均较低,感染年龄趋于年轻化和老年化。Objective. To explore the prevalence of human papillomavirus (HPV) and genotypes distribution in Tibetan women living in plateau area. Methods Genotypes of HPV from 7051 Tibetan women's cervix swabs were detected u sing flow-through hybridization and gene chip teohnology. 15 kinds of high-risk types were HPV16, 18, 31, 33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66, 68), and 6 kinds of low-risk types were HPV6, 11, 42, 43, 44, CP8304. Results: The total positive rate of HPV among 7051 Tibetan women was 19.50%, and 21 subtypes of HPV were detected, of which HPV16 (32.29%) was the most common one, followed by HPV58 (19.64%), HPV52 (10.25%), HPV53 (7.42%) and HPV18(5.96%). The infection rate of high--risk genotype, single low- risk one were 93.60% (1287/1375) and 6.400/00 (188/1375). The single and multiple infection rates were 88.58% (1218/1375) and 11.49 % (158/1375). There were two peaks of infection among different age groups., the first one in women less than 25 years old(24.23%), and the second one in women not less than 55 years old(30.54%). Conclusion.. The common infection genotypes are HPV16, HPV58, HPV52, HPV53 and HPV18 in Tibetan women living in plateau area, and the highrisk genotype as well as the single infection are primary. There is a low infection rate in simple low-risk genotype and multiple infection, and the age at infection tends to be younger and older.
