螺旋CT双期增强扫描对可疑胰头癌的鉴别诊断

摘　　要：目的 :探讨螺旋CT双期增强扫描对可疑胰头癌的鉴别诊断价值。方法 :对 49例经超声和临床拟诊的胰头癌行胰腺的螺旋CT双期增强扫描 ,动脉期和门静脉期延迟扫描时间分别为 2 5秒和 6 0秒。所有病例经病理或临床追踪诊断。结果 :49例中 ,胰头变异 5例 ,胰头周围淋巴结病 6例 ,慢性胰腺炎 7例 ,胰头癌 31例。其中 46例 (94% )诊断正确 ,1例胰头周围淋巴结结核和 2例慢性胰腺炎误诊为胰头癌。结论 :螺旋CT双期增强扫描对可疑胰头癌的鉴别诊断有很高的价值。Objective:To study the value of dual-phase contrast enhanced spiral CT in the diagnosis of suspicious pancreatic head cancer.Methods:49 cases of suspicious pancreatic head cancer based on clinical symptoms and ultrasound were scaned by contrast enhanced spiral CT,the scan delay time of the arterial and portal venous phase was 25s and 60s respectively.All the cases had pathological or clinical follow-up results.Results:There were 5 cases of malformation of pancreatic head,6 lymphadenopathy around pancrea t ic head,7 chronic pancreatitis,31 pancreatic head cancer.46 of 49 cases(94%)were accurately diagn osed.One lymph nodes tuberculosis and two chronic pancreatitis were misdiagnosed . Conclusion:Dual-phase contrast enhanc ed spiral CT has high value for differential diagnosis of suspicious pancreatic head cancer.
