先天性桡侧序列缺损的二维和三维超声产前诊断

摘　　要：目的探讨产前诊断先天性桡侧序列缺损(RRD)的二维和三维超声特征,以提高其早期诊断率。方法回顾性分析产前超声诊断并经引产证实的36例先天性RRD胎儿的声像图表现及随访结果,总结先天性RRD的二维及三维超声图像特点。结果 36例先天性RRD胎儿中,桡骨完全缺失26例,其中双侧14例,单侧12例;桡骨发育不全或部分缺如10例,其中双侧3例,单侧7例。主要超声特征为患侧手呈桡偏的"柺棒手",其前臂仅见1根长骨或者可见2根长骨但其中1根短小。结论桡偏"柺棒手"是超声早期诊断RRD的重要诊断线索,二维超声即可发现RRD,三维超声可提供形态学信息,有利于明确诊断。Objective To investigate the characteristic of 2D and 3D ultrasound in prenatal diagnosis of inborn radial ray defects（ RRD）,in order to improve the early diagnosis rate. Methods The sonographic appearance and follow-up results of 36 cases of inborn RRD confirmed by prenatal ultrasound diagnosis and artificial labor were analyzed retrospectively. The 2D and 3D sonographic features of inborn RRD were summarized. Results Among 36 cases of inborn RRD,the complete absence of radius in 26 cases（ bilateral absence in 14 cases,unilateral absence in 12 cases）,agenesis or partial absence in 10 cases（ bilateral hypoplasia in 3 cases,unilateral hypoplasia in 7 cases）. The main sonographic features of inborn RRD were hand anomalies with fixed postures which named radial club hand,and the affected forearm had only one bone or two bones in which one bone was very short. Conclusion Radial club hand is the important ultrasonic appearance of early RRD,RRD can be diagnosed accurately by 2D ultrasound,3D ultrasound can provide morphological characteristics to confirm the diagnosis.
