青光眼术后浅前房的原因分析及粘弹性物质在治疗中的作用

摘　　要：目的 :探索青光眼手术后浅前房的发生原因及粘弹性物质在治疗顽固性前房不形成的效果。方法 :回顾分析青光眼手术患者6 4例 81眼发生浅前房的比率、发生时间、类型及处理方法 ,分析不同青光眼类型和手术方式对术后浅前房的影响。结果 :青光眼术后浅前房 43眼占 5 3.1%。闭角型青光眼术后发生率较高 (占 40 .7% ) ,青光眼滤过手术后易引起浅前房。浅前房发生在术后 1～ 7天 ,房水滤过过强占 5 3.5 % ,脉络膜剥离占 14.0 % ,伤口渗漏占 9.3%。 1眼行结膜瓣修复 ,1眼通过脉络膜上腔放液得以恢复。 6眼因顽固性前房不形成行前房注入透明酸钠 ,其余均保守治疗。结论 :青光眼滤过手术后浅前房发生率高 ,其最常见的原因是房水滤过过强。大多数浅前房通过非手术治疗 。Objective:To investigate the cause of shallow anterior chamber after glaucoma surgery and effect of treating sustained chamber disappearance with Healon.Methods:Retrospectively examine ratios,times,types and managements of shallow anterior chamber in 81 eyes of 64 patients after glaucoma surgery,and analyzed influences of shallow anterior chamber in type of glaucoma surgery,and analyzed influences of shallow anterior chamber was 53.1%.The incidence of closed glaucoma was higher(40.7%).The filtration surgery was more likely to cause shallow anterior chamber than other kind of surgery.A large part of shallow anterior chambers was caused by excessive aqueous homor runoff(53.5%).It may appear 1 7days of post operation.The most of them were managed with non surgical therapy.6 eyes of sustained chamber disappearance were treated by injecting Healon into chambers.Conclusion:The incidence of filtration surgery causing shallow anterior chamber was higher.The effects of the treatment with Healon were proved quite good.
