摘  要:目的探讨正常口腔粘膜、口腔扁平苔藓、口腔白斑、口腔鳞癌粘膜组织中Bak蛋白的表达及细胞凋亡与口腔黏膜病变发生发展的相关性。方法取正常黏膜组织30例、口腔扁平苔藓组织60例(白纹型30例、糜烂型30例)、口腔白斑组织30例(轻、中、重度非典型性增生3个亚组各10例)、口腔鳞癌30例(Ⅰ级、Ⅱ级、Ⅱ-Ⅲ级各10例)免疫组化测定凋亡蛋白Bak表达量,原位末端转移酶标记法(TUNEL)标记凋亡细胞,检测正常口腔粘膜、口腔扁平苔藓、口腔白斑、口腔鳞癌粘膜组织固有层淋巴细胞的细胞凋亡指数。结果Bak蛋白在正常对照组、白纹型口腔扁平苔藓、糜烂型口腔扁平苔藓组织固有层淋巴细胞浸润带阳性率依次增高。口腔白斑固有层中,Bak蛋白表达量随细胞异常增生程度的增加而减低。口腔鳞癌Ⅰ级、Ⅱ级、Ⅱ到Ⅲ级组织固有层淋巴细胞浸润带阳性率依次增高。其中鳞癌Ⅱ级与鳞癌Ⅱ到Ⅲ级间,差异无统计学意义(P=1.56),其余差异有统计学意义(P〈0.05)。口腔黏膜癌前病变、口腔鳞癌组织细胞凋亡指数明显高于正常黏膜对照组,且其随疾病的进展及病变恶化发展逐渐增高,鳞癌Ⅱ级与鳞癌Ⅱ到Ⅲ级两组间统计学分析无差异(P=1.01),其余组间均有具统计学意义的差异(P〈0.05)。结论Bak蛋白在口腔扁平苔藓、口腔鳞癌组织的表达皆随病变进展加重而呈现增加趋势,而口腔白斑中,Bak蛋白随异常增生程度的增高而表达减低。随口腔组织病变加重,凋亡指数呈明显递增趋势。Objective To investigate the expression of Bak protein in normal mouth mucosa, oral lichen planus, oral leukoplakia and oral squamous carcinoma mucosal tissue and the correlation between cell apoptosis and the occurrence and development of oral mucosa lesion. Methods 30 normal mucous membranes, 60 oral lichen planus tissue (30 white stripes types and 30 erosion types), 30 oral leukoplakia tissue (each of the 3 subgroups of light, moderate, heavy atypical hyperplasia has 10 cases), 30 oral sqnamous cell carcinoma (each of the Ⅰ grade,Ⅱ grade, Ⅱ -Ⅲ grade has 10 cases) have been selected to assay by immunohistochemical staining. In situ DNA nick end labeling (TUNEL) was used to label theapoptotie cells and detect the lymphocytic apoptotie indexes of normal mouth mucosa, oral lichen planus, oral leukoplakia and lamina propria of oral squamous cell carcinoma mucosal tissue. Results The positive rates of lymphoeytes infiltration band of Bak protein increased successively in normal control group, oral lichen planus of white stripes types and lamina propria of oral lichen planus tissue of erosion types. In the lamina propria of oral leukoplakia, the expressions of Bak protein decreased as the cell paraplasm levels increased. The positive rates of lymphoeytes infiltration band of increased successively in tissue lamina propria of grade Ⅰ, grade Ⅱ , intergrade Ⅱ to m of oral squamous cell carcinoma. The differences were not significant between Grade Ⅱ and intergrade Ⅱ to Ⅲ of squamous carcinoma (P= 1.56), the other differences had statistical significance (P〈0.05). Oral mucosa precancerous lesion occurrences and the cell apoptotic indexes of oral squamous cell carcinoma tissue were significantly higher than those of the normal mucosa control group, and their progression of disease and the deterioration of lesion development gradually increased.
