摘 要:目的:探讨乳腺浸润性导管癌(infiltrating ductal breast carcinoma,IDCA)患者α1-抗胰蛋白酶(α1-antitrypsin,α1-AT)表达水平与临床病理因素的相关性。方法:采用酶联免疫法(ELISA)和免疫组化法(IHC)测定2010-01-31-2012-06-30山东省医学科学院附属医院50名正常对照组及176例浸润性导管癌患者(IDCA组)血清和癌组织中α1-AT表达水平,同时记录患者年龄、TNM分期、肿瘤大小、淋巴结转移情况、肿瘤分化程度、HER-2和激素受体状态等临床病理资料,采用秩相关分析患者血清中α1-AT水平与其相关性,分析各组免疫组化染色综合评分差异。结果:IDCA组Ⅰ、Ⅱ、Ⅲ和Ⅳ期中血清α1-AT平均值分别为(3.04±0.72)、(3.58±0.45)、(4.18±1.33)和(5.02±1.21)ng/mL,明显高于对照组的(1.73±0.62)ng/mL,各组间差异均有统计学意义,P<0.05。秩相关分析显示,IDCA患者血清α1-AT水平与TNM分期(r=0.756,P<0.001)、CA15-3水平(r=0.699,P=0.024)呈正相关,与其他因素无关。免疫组化染色综合评分显示,IDCA患者Ⅰ、Ⅱ、Ⅲ和Ⅳ期中α1-AT蛋白综合评分分别为3.47±0.88、5.36±1.26、9.33±2.15和12.38±2.43,明显高于对照组的1.24±0.17,各组间差异均有统计学意义,P<0.05。经Spearman相关分析显示,IDCA组肿瘤组织α1-AT蛋白IHC染色综合评分与TNM分期呈正相关,分期越高,表达越强,rs>0,P<0.05。结论:IDCA患者肿瘤组织α1-AT蛋白表达水平较正常对照组高,且与TNM分期相关,分期越高,表达越强。OBJECTIVE: To investigate correlativity of the expression of α1-AT levels with the clinical pathological factors in IDCA patients. METHODS: ELISA and IHC were performed to measure the expression levels of α1-AT of sera and tissues in both 50 healthy women with normal menstrual cycle and 176 IDCA patients. The clinical pathological factors including age,TNM stage, tumor size, metastasis of lymph node, tumor differentiation, HER-2 status, hormone receptor status and other else were recorded and with which rank correlation analysis was used to determine correlativity of the ex- pression of α1-AT levels in the sera. ANOVA was performed to determine the difference of the staining scores for each group. RESULTS: The average serum α1-AT concentrations in stageⅠ、Ⅱ、Ⅲ,Ⅳ were (3.04±0. 72), (3.58±0.45), (4.18 ± 1.33), (5.02 ± 1.21) ng/mL respectively, which were evidently higher than that in the normal controls (1.73 ± 0.62 ng/mL),and the difference in each group was significant(P〈0.05). By rank correlation analysis, the expression levels of α1-AT of sera in IDCA patients were correlated positively with TNM stage (r=0. 756 ,P〈0. 001)and the concentrations of CA15-3(r= 0. 699, P = 0. 024). By semiquantitative analysis, the staining scores of stage Ⅰ、Ⅱ、Ⅲ,Ⅳ were 3.47± 0.88,5.36 ± 1.26,9.33 ± 2.15,12.38±2.43 respectively, which were evidently higher than that in normal controls 1.24 ± 0.17, and the difference in each group was significant (P〈0.05). By Spearman analysis, the staining score of IDCA groups were correlated with TNM stage(rs 〉0, P〈0.05). α1-AT expression was increased with the progression of TNM staging. CONCLUSION: Each staining score of IDCA groups is higher than that in normal control, which is correlated with TNM stage and α1-AT expression is increased as the TNM staging progression.
