经皮下注射超声造影剂研究luminal A型乳腺癌前哨淋巴结的增强模式

摘　　要：目的经皮下注射超声造影剂研究luminal A型乳腺癌前哨淋巴结的增强模式。方法对女性乳腺癌患者术前经肿块周围皮下注射超声造影剂SonoVue探查前哨淋巴结并标记,术后肿块做免疫组织化学检查,选择术后免疫组化结果为luminal A型的53例患者的前哨淋巴结超声造影图像进行回顾性分析。结果 53例luminal A型乳腺癌病例中,超声造影检查有前哨淋巴结转移组,12例为非均匀增强,3例为均匀增强;无前哨转移组,34例为均匀增强,4例为非均匀增强。超声造影发现luminal A型乳腺癌前哨淋巴结转移率为28.3%(15/53);超声造影检查的灵敏度为68.8%,特异度为89.2%,准确度为83.0%。结论经皮下注射超声造影剂luminal A型乳腺癌前哨淋巴结超声造影增强模式有一定的规律,即有转移组主要表现为非均匀性增强,无转移组主要表现为均匀性增强。Objective To research the enhanced mode of contrast-enhanced ultrasound by Percutaneous injection of ultrasound contrast agents in sentinel lymph node metastasis of luminal A-type breast cancer.Methods A total of 53 luminal A-type breast cancer cases were performed to retrospectively analysis the enhanced mode of the sentinel lymph nodes after injection of ultrasound contrast agents preoperative and marked them after surgery,the resection of breast cancer were performed by immune histological examination.Results Among the 53 cases of breast cancer patients,12 cases for inhomogeneous enhancement and 3 cases for homogeneous enhancement in the group of sentinel lymph node metastasis by the characteristic of ultrasound angiography; 34 cases for homogeneous enhancement and 4 cases for inhomogeneous enhancement in the group of no sentinel lymph node metastasis.Luminal A-type sentinel lymph node of breast cancer metastasis rate by ultrasound angiography was 28.3 % （15/53）,sensitivity,specificity and accuracy of axillary lymph node metastasis by contrast-enhanced ultrasound is 68.8%,89.2%,83.0% respectively.Conclusions Percutaneous injection of ultrasound contrast agents has the certain regularity in sentinel lymph node of luminal A-type breast cancer,the group of sentinel lymph node metastasis was mainly for inhomogeneous enhancement,the group of no sentinel lymph node metastasis was mainly for homogeneous enhancement.
