摘 要:目的 探讨声辐射力脉冲成像技术(acoustic radiation force impulse imaging,ARFI)技术在无创定量评价酒精性肝病(al-coholic liver disease,ALD)肝纤维化程度的临床应用价值.方法 对108例ALD患者应用ARFI技术对肝脏进行弹性测量,所有患者同时进行超声引导下肝脏穿刺活检,以病理结果为金标准,对肝脏的ARFI测值与肝纤维化程度的相关性进行统计分析.结果 ALD患者肝纤维化分期S0~S4的ARFI测值分别为(1.14±0.72) m/s,(1.26±0.23) m/s,(1.50±0.34) m/s,(2.08±0.76) m/s和(1.98±0.39) m/s,组间比较差异具有统计学意义(P<0.05).肝脏ARFI测值与ALD肝纤维化分期呈正相关,Spearman相关系数为0.60 (P=0.001).ARFI技术诊断ALD肝纤维化程度≥S1,≥S2,≥S3和=S4的受试者工作特征曲线下面积分别为0.848、0.803、0.840和0.836.结论 ARFI技术可较准确的无创定量评价ALD肝纤维化程度.Objective To investigate the diagnostic value of the acoustic radiation force impulse(ARFI)technology for the alcoholic liver disease (ALD).Methods The study subjects were composed with 108 patients with ALD.The measurements of liver stiffness by ARFI technology using a ultrasonic instrument named Acuson S2000 and with liver biopsy as the gold standard.Results The liver stiffness at ARFI sonoelastography were (1.14±0.72)m/s m/s for the patients with S0.(1.26±0.23)m/s for those with S1,(1.50±0.34)m/s for those with S2,(2.08±0.76)m/s for those with S3,and (1.98±0.39)m/s for those with S4,respectively.The ARFI values were significantly different (P〈0.05) among the different stage of liver fibrosis.Liver stiffness at ARFI was significantly correlated with liver fibrosis stage in ALD (r=0.60,P=0.001).Using the ARFI technology for the assessment of liver fibrosis in ALD patients,the area under of receiver operating characteristic AUROC for the patients with≥S1,≥S2,≥S3 and =S4were 0.848,0.803,0.840 and 0.836,respectively.Conclusions ARFI technology can be more accurate noninvasive quantitative evaluation of ALD liver fibrosis atage.
