肠易激综合征患者直肠肛管测压的研究

摘　　要：研究肠易激综合征 (IBS)患者直肠肛管压力的变化。应用美国Sandhill公司提供的BIOLAB消化道压力检测仪及固态压力传感器导管 ,对 43例明确诊断为IBS患者及 2 0名正常人进行直肠肛管压力检测 ,观察各参数的改变。结果IBS患者腹泻组的FSV、DSV、DUSV、MTV均较正常对照组低 ,其中DSV、DUSV和MTV与对照组比较有显著性差异 (P <0 0 1) ,便秘组的FSV、DSV、DUSV及MTV均较对照组高 ,其中FSV、DSV和DUSV和对照组比较有显著性差异 (P <0 0 1) ;IBS患者腹泻组的RRP、ARP及MSQP较对照组高 ,ARPD较对照组低 ;便秘组的RRP、ARP及ARPD均较对照组高 ,MSQP较对照组低 ;ISB患者腹泻组、便秘组的ARIR量均高于对照组且有显著性差异 (P <0 0 1) ;IBS患者腹泻组的顺应性降低而便秘组的顺应性增高 (P <0 0 1)。To investigate fecal continence in patients with irritable bowel syndrome (IBS), studies were performed in 43 patients with IBS and 20 healthy subjects. Anorectal pressure was measured with BioLAB monitor of digestive tract and anorectal catheter with solid state manometric transducers. The results were analyzed by computation programs of American Sandhill Company. The sensation of defecation volume (DSV),sensation of defecation urgent volume (DUSV) and maximal tolerable volume (MTV) in the patients with diarrhea predominant type IBS (Dp-IBS) were significantly lower than healthy subjects (P<0.01). The first sensation volume (FSV), DSV and DUSV in the patients with constipation predominant type IBS (Cp-IBS) were significantly higher than healthy subjects (P< 0.01).The rectal resting pressure (RRP) and anal resting pressure (ARP) in patients with Dp-IBS were significantly higher than healthy subjects (P<0.01). The RRP and maximal squeezing pressure (MSQP) in the patients with Dp-IBS were significantly higher than healthy subjects (P<0.01), and MSQP in the patients with Cp-IBS was significantly lower than healthy subjects (P<0.01). The rectl anal inhibitory reflex (RAIR) volume in the patients with Cp-IBS and Dp-IBS were significantly higher than healthy subjects (P<0.01). The rectal compliance (RC) in the patients with Cp-IBS was significantly higher and that in patients with Dp-IBS was significantly lower than healthy subjects (P<0.01).The results suggested the ability of fecal continence in the patients with IBS was abnormal.
