摘 要:目的 :研究人肺癌组织中细胞增殖状态、凋亡水平与临床病理生理特征之间的关系 ,探讨肿瘤细胞增殖与凋亡的关系。方法 :应用免疫组化法检测了 16 6例肺癌组织和 37例肺良性病变组织中核增殖相关抗原Ki 6 7的表达水平 ,应用TUNEL法检测其细胞凋亡水平。结果 :(1)肺癌组织中Ki 6 7指数显著高于肺良性病变组织(P <0 0 1) ,而凋亡指数显著低于对照组 (P <0 0 1) ;(2 )肺癌组织中Ki 6 7指数增高与肺癌组织学类型、细胞分化程度显著相关 (P <0 0 1) ,而与患者P -TNM分期、淋巴结转移程度、吸烟与否无明显关系 (P >0 0 5 ) ;(3)肺癌组织中凋亡指数与组织学类型、细胞分化程度无关 (P >0 0 5 ) ,而与患者P -TNM分期、淋巴结转移状态显著相关 (P <0 0 1) ;(4)在Ⅰ、Ⅱ期病例中肺癌细胞凋亡指数与Ki 6 7指数呈显著正相关 (RR =0 2 14,P <0 0 5 ) ,而在Ⅲ、Ⅳ期病例中没有发现相关 (P >0 0 5 )。结论 :Ki 6 7指数反映肺癌细胞固有的增殖能力 ,可能与组织学类型及细胞分化程度密切相关 ,而与患者病期无明显关系 ;凋亡指数可能反映机体与肿瘤细胞的相互作用 ,与患者病期及淋巴结转移状态密切相关。Objective:To investigate the relationship between cell proliferation,apoptosis and clinicopathological characteristics in primary lung cancer.Methods:The expression of Ki 67 was detected in 166 as cancer samples and 37 benign lung lesion tissues as control by immunohistochemistry,and their apoptosis level was assessed by TUNEL.Results:(1)the Ki67 index(KI)was significantly higher in lung cancer tissues than in that of benign lesion tissues( P <0 01),and the apoptosis index(AI)in cancer tissues was significantly decreased comparing with the control( P <0 01);(2)the KI in lung cancer tissues was closely related to histological classification and cancer cell differentiation( P <0 01),otherwise,no significant relationship was found between KI and P TNM stages,lymph node involvement and smoking or not in lung cancer patients( P >0 05);(3)the AI of lung cancer tissues is significantly related to P TNM stages,lymph node involvement( P <0 01);(4)there was a positive relationship between the Al and Kl in cancer tissues of stage Ⅰ and stage Ⅱ patents( P <0 01),but no such relationship was found in stage Ⅲ or stage Ⅳ patients.Conclusion:KI was a favorable marker to proliferation of lung cancer cells,which migh have close relationship with the histological classification and cell differentiation,and AI might reflect the interaction betwen the tumor cells and the patient,and close inter-relation betwen the stages of the disease and condition of the lymphnode metastasis.capacity of body to erase the malicious cell.
