摘 要:目的 比较银杏叶提取物(Ginkgo biloba extract,EGB)凝胶与盐酸米诺环素软膏(派丽奥)治疗牙周炎的临床疗效及其对牙周可疑致病菌的抑制作用,以期为牙周局部用药提供参考.方法 从首都医科大学附属北京朝阳医院口腔科就诊的中重度牙周炎患者中选取30例,按就诊顺序分为EGB试验组(试验组)和盐酸米诺环素软膏自身对照组(对照组),每组30例,60颗牙,120个位点.常规进行龈上洁治和龈下刮治治疗,分别在治疗前及治疗后1周、2个月和4个月采集龈下菌斑样本,用PCR法检测4种主要牙周致病菌牙龈卟啉单胞菌(Porphyromonas gingivalis,Pg)、福赛类杆菌(Bacteroides forsythus,Bf)、中间普氏菌(Prevotella intermedia,Pi)及齿垢密螺旋体(Treponema denticola,Td).在治疗前及治疗后3、6个月检查反应牙龈炎(菌斑指数、出血指数)及牙周炎(探诊深度、附着丧失)的临床指标.对4种牙周致病菌的检出率采用卡方检验、对菌斑指数和出血指数采用方差齐性检验及t检验、对探诊深度和附着丧失进行重复测量的方差分析,检验水准为单侧α =0.05.结果 治疗前4种牙周致病菌的检出率分别为试验组:Td:83.3%,Bf:95.0%,Pi:80.0%,Pg:81.7%;对照组:Td:83.3%,Bf:95.0%,Pi:80.0%,Pg:81.7%,除治疗后1周时试验组Pg的检出率(56.7%)与对照组(53.3%)相比差异有统计学意义外(P<0.01),其余时间点试验组与对照组相比差异均无统计学意义(P>0.05);试验组与对照组菌斑指数和出血指数在治疗后各时间点相比差异均无统计学意义(P>0.05);在治疗后3、6个月,两两比较结果显示探诊深度随治疗时间延长而减少,治疗后6个月时两组探诊深度[试验组:(3.5±0.5)mm,对照组:(3.2±0.4)mm]差异有统计学意义(P=0.00);附着丧失也随治疗时间延长而减少,治疗后6个月时两组间相比[试验组:(4.5±0Objective To compare the clinical effect of Ginkgo biloba extract gel (Ginkgo biloba extract,EGB) and minocycline hydrochloride (Periocline) on periodontitis and their inhibition on putative periodontal pathogens.Methods Thirty patients with moderate-to-severe periodontitis were selected.The patients were divided into an experimental group and a positive control group (minocycline hydrochloride) Supragingival and subgingival scaling were performed on all patients.Subgingival plaque samples were collected before treatment,1 week,2 months and 4 months after treatment.The four major periodontal pathogens Porphyromonas gingivalis (Pg),Bacteroides forsythus (Bf),Prevotella intermedia (Pi),Treponema denticola(Td) were detected by polymerase chain reaction.Clinical indexes plaque index (PLI),bleeding index(BI) and probing depth(PD),attachment loss (AL) were examined before treatment,3 months and 6 months after treatment.The results were statistically analyzed.Results The detection rates of the 4 periodontal pathogens were Td (83.3%),Tf(95.0%),Pi (80.0%),Pg(81.7%) in experimental group and Td (83.3 %),Tf(95.0%),Pi (80.0%),Pg (81.7%) in control group before treatment.The detection rates in experimental group were not significantly different with those in control group after treatment,except for the detection rate of Pg 1 week after treatment (P < 0.01,the detection of Pg was 56.7% in experimental group and 53.3% in control group 1 week after treatment).The PLI and BI were not significantly different between experimental group and control group after treatment (P > 0.05).The difference was statistically significant at 6 months after treatment [(3.5 ± O.5) mm for experimental group and (3.2 ± 0.4) mm for control group,P =0.00].The mean of AL decreased with time.The difference was statistically significant at 6 months after treatment[(4.5 ±0.4) mm for experimental group and (4.3 ±0.4) mm for control
