早期康复对脑卒中偏瘫合并心理障碍疗效观察

摘　　要：目的探讨综合康复治疗对脑卒中偏瘫合并心理障碍患者的临床价值。方法选择脑卒中导致偏瘫合并心理障碍患者112例,随机分为治疗组和对照组各56例,两组患者入院后均行神经内科常规治疗、肢体功能康复训练及低频电刺激治疗;治疗组在康复治疗的基础上再进行认知功能和心理治疗。在患者治疗前和治疗1个月时应用简式Fugl-Meyer运动功能积分(FMA)、简易精神状态检查(MMSE)、汉密尔顿抑郁量表(HAMD)进行评定。结果治疗1个月后,治疗组FMA和MMSE评分明显提高(t=-7.93,P<0.01,t=2.98,P<0.01),HAMD评分显著下降(t=-7.93,P<0.01)。结论对于脑卒中偏瘫合并心理障碍的患者,在进行早期康复训练的同时,积极开展心理治疗,不仅有利于患者的认知功能和心理状态的改善,同时也能促进其肢体运动功能的恢复。Objective To explore the clinical value of multidisciplinary rehabilitation to stroke patients with hemiplegia following psychological disorders .Methods A total of 112 stroke patients with hemiplegia following psychological disorders were randomly divided into two groups：56 cases for treatment group and 56 cases for control group .The two groups received nerve routine medical treatment , limb function rehabilitation training and low-frequency electrical stimulation after admission .Cognitive and psychological therapy was ad-ditionally used in the treatment group.The patients were assessed by Fugl -Meyer Assessment（FMA）,Mini-Mental State Examination （ MMSE） and Hamilton Rating Scale for Depression （ HAMD） before and after treatment for 1 month.Results After 1 month for interven-tion,the scores of FMA and MMSE increased significantly （（t=-7.93,P〈0.01;t=2.98,P〈0.01）but the scores of HAMD decreased significantly（t=-7.93,P〈0.01）in the treatment group.Conclusion Early rehabilitation training combined with psychological therapy can promote not only the improvement of cognitive function and psychological state but also limb motor function recovery for stroke pa -tients with hemiplegia following psychological disorders .
