长期居家老年人压疮发生风险与自理能力研究分析

摘　　要：目的描述长期居家老年人压疮发生状况,明确日常生活自理能力和老年人压疮之间的关系。方法运用分层整群抽样方法,采用日常生活能力量表等对长期居家的909位老年人进行调查。结果长期居家老年人压疮发生率25.77%;其中Ⅰ期压疮33.33%,Ⅱ期压疮49.57%;压疮组老年人ADL、IADL总分为(47.19±24.20)分和(4.40±5.20)分,明显低于无压疮组,差异有显著意义(P<0.05);两组老年人ADL及IADL分级差异也有显著意义(P<0.05)。Logistic回归分析显示,中度及重度功能障碍、失能是压疮发生的影响因素。结论长期居家老年人压疮发生率高,以Ⅱ期压疮为主;日常生活活动能力和工具性日常生活活动能力是其压疮发生的重要影响因素。居家护理应采取针对性的预防和干预。Objective To describe the incidence of pressure ulcer in the elderly for long-term care at home, and toclearly the relationships between self-care ability of daily life（ADL） and pressure ulcers in the elderly. Method Strat-ified cluster sampling method and activity of daily living scale was used to investigate 909 elderly with long-term careat home. Result The pressure sore occurred 25.77 % among in-home long-term care elderly patients, among whichstage I pressure ulcers was 33. 33%, stage 11 pressure ulcer 49. 57%; Pressure ulcer group aged ADL, IADLscores were 47.19 ±24.20, 4.40 ±5.20, it＇s significantly lower than that of the no pressure ulcer group, Therewas significant difference （P〈0.05）; Logistic regression analysis showed that moderate and severe dysfunction,disability were important influence factors of pressure sore. Conchmion The incidence of pressure sore in in-homeelderly patients with long-term care is high, stage 11 pressure ulcer is common, and ability and instrumental activi-ties of daily living activities of daily life is an important factor influencing the pressure ulcer. Home care should betaken for prevention and intervention.
