摘 要:目的探讨全数字化钼靶对不可扪及乳腺BI-RADS4病变的微创活组织检查的临床价值。方法回顾性分析2010年11月至2013年11月在本院对不可扪及乳腺BI-RADS4病变接受全数字化钼靶的微创活组织检查的51例(55处病灶,其中4例为双侧),55处病变的临床、影像及病理资料。微创活检组织接受小焦点放大技术检测确认活检可靠。结果全组55处病灶钼靶表现为结节7例,局灶性致密1例,结构扭曲1例,无定型钙化46例,全部在全数字化钼靶定位下活检,全部被准确活检,51例(55处病灶)中,恶性病变14处,其中导管原位癌6处,浸润性导管癌8处,总体恶性率25.5%;良性病变41处,其中导管内乳头状瘤1处,纤维瘤2处,炎症1处,硬化性腺病37处,占74.5%。结论不可扪及乳腺BI-RADS4病变患者经钼靶定位下活检是一种微创、简便、实用的方法,易于推广。Objective To Explore the clinical value of minimally invasive biopsy guided by full digital mammography for non palpable BI-RADS 4 lesions. Methods Fifty one patients with 55 lesions (4 patients with bilateral lesions)were collected in the study. The lesions were BI RADS4 lesions on digital mammography images. All the lesions were non palpa- ble lesions and performed minimally invasive biopsy. The biopsy tissues were performed small focal amplification technologyto confirm the biopsy and reliable. Results On Mammography images, 55 lesions showed nodules in 7 cases, focal dense in 1 case, structure distortion in 1 case, amorphous calcification in 46 cases. After biopsy, the pathological results showed 14 cases (25.5%)of malignant lesions , included intraductal carcinoma in 5 cases, ductal carcinoma in situ in 1 case, in- vasive ductal carcinoma in 8 cases, 41 cases (74.5%) of benign lesions, included intraductal papilloma in I case, flbroma in 1 case, inflammation in 2 cases, sclerosing adenosis in 37 cases. Conclusion Biopsy guided by mammography is a minimally invasive and practical method for Non palpable BI RADS4 lesions.
