吻合器在胃癌经腹全胃切除、消化道重建术中的应用

摘　　要：目的　总结国产管状吻合器在胃癌经腹全胃切除术中应用的临床效果。方法　对 15 5例胃癌经腹做全胃切除 ,应用国产管状吻合器完成空肠侧侧吻合 92例 ,端侧Y型吻合 6 3例。结果　 15 3例一次性吻合成功 ,2例吻合器吻合失败改行剖胸再吻合。无吻合口瘘及出血 ,术后随访 0 .5～ 2年 ,无吻合口狭窄发生。结论　熟悉国产吻合器性能 ,操作得当 。Objective To study the clinical value of domestic tubular stapler in transabdominal total gastrectomy for gastric carcinoma.Methods 155 cases of gastric cancer were treated with transabdominal total gastrectomy.92 cases of side to side Braun anastomoses and 63 cases of Y type anastomoses were accomplished with domestic stapler.Results 153 cases were one shotly anastomosed,but two cases had to be reanastomosed through chest.No anastomotic leakage and hemorrhage was discovered in perioperative period.After a follow up of six monthes to two years,no anastomotic stenosis occured.Conclusion Being familiar with the function of domestic tubular stapler and operating appropriatly will lead to a satisfactory outcome.
