中药肝豆灵对肝豆状核变性患者尿微量蛋白的影响

摘　　要：目的观察肝豆灵对肝豆状核变性(hepatolenticular degeneration,HLD)患者早期肾脏损害的干预作用。方法将70例HLD患者随机分成治疗组(n=35)和对照组(n=35),对照组接受常规保肝排铜治疗,治疗组加用中药肝豆灵口服,连续治疗4周。治疗前后分别观察两组尿免疫球蛋白G(urinary immunoglobulin G,UIgG)、尿转铁蛋白(urinary transferrin,UTRF)、尿微量白蛋白(microalbuminuria,MAU)、尿α1微球蛋白(urinary alpha 1microglobulin,Uα1M)、尿β2微球蛋白(urinary beta 2microglobulin,Uβ2M)的变化。结果治疗后治疗组UTRF、MAU、Uα1M和Uβ2M均较治疗前显著降低(P<0.05,或P<0.01),而对照组仅UTRF、Uα1M和Uβ2M显著降低(P<0.05,或P<0.01)。治疗后治疗组Uα1M下降程度显著大于对照组(P<0.05)。讨论肝豆灵对HLD患者肾脏早期损害有一定干预作用。Objective To observe the effect of the traditional Chinese medicine Gandouling on early renal damage in patients with hepatolentieular degeneration （HLD）. Methods Seventy HI.D patients were randomly divided into treatment group （n=35） and control group （n=35）. The control group received routine treatment for liver protection and copper discharge, while the treatment group received Gandouling in addition to the routine treatment. The course of treatment was 4 weeks for both groups. The changes in ufinery immunoglobulin G （UIgG）, urinary transferrin （UTRF）, microalbuminuria （MAU）, urinary alpha 1 microglobulin （Uα1M）, and urinary beta 2 microglobulin （Uβ2M） were evaluated and compared between the two groups. Results After treatment, the treatment group showed significant decreases in UTRF, MAU, Uα1M, and Uβ2M （P〈0.05 or P〈0.01）, while the control group showed significant decreases in UTRF, Uα1M, and Uβ2M （P〈0.05 or P〈0.01）. The treatment group had a significantly higher decrease in Uα1M than the control group （P〈0.05）. Conclusion Gandouling can reduce the early renal damage in HLD patients.
