循证护理对预防脑卒中昏迷患者胃肠道及呼吸系统感染效果研究

摘　　要：目的 观察循证护理对预防脑卒中昏迷患者并发胃肠道及呼吸道感染的疗效,以指导临床脑卒中昏迷病人的护理实践.方法 选取2009年4月～2013年5月来我院就诊的脑卒中昏迷患者82例,依据护理方法的不同将患者分为循证组和常规组各44例,循证组在常规治疗基础上对患者进行循证护理,常规组给予常规护理.结果 治疗前两组患者的检测指标无明显差异（P＞0.05）;治疗4周后,循证组的WBC、ESR、痰定量培养分离致病菌数、胸部CT示肺部阴影例数、血细菌培养阳性例数及鼻饲管引流液细菌培养阳性例数均明显低于常规组（P＜0.05）;循证组患者的脂类吸收率明显高于常规组（P＜0.05）.结论 应用循证护理方法对脑卒中昏迷患者进行护理可明显预防患者并发肺部及胃肠道感染的发生,为患者的康复奠定良好基础.Objective To explore the effects of prevention of gastrointestinal and respiratory infections on evidencebased nursing for coma patients with stroke to guide clinical stroke coma patient care practices.Methods 82 coma patients with stroke were selected.The patients were divided into evidence-based group and conventional group according to the different methods of care for patients.There were 41 cases in each group.The 41 cases of evidence-based group were taken evidence-based nursing and the 41 cases of conventional group were taken conventional nursing.Results The detected index of the two groups of patients showed no significant differences before treatment （P〉0.05）.The WBC,ESR,the number of bacteria isolated quantitative culture of sputum,chest CT showed pulmonary shadows number of cases,blood culture positive cases and nasogastric tube draining fluid bacterial culture positive cases of the evident-based group were significantly lower than that of the conventional group after 4 weeks of treatment （P〈0.05）.The lipid absorption in patients of evidence-based group was significantly higher than the conventional group （P〈0.05）.Conclusion The application of evidence-based nursing to patient care in a coma stroke can prevent the pulmonary and gastrointestinal infections significantly and lay a good foundation for the rehabilitation of patients.
