川芎嗪联合尼莫地平防治破裂颅内动脉瘤栓塞术后脑血管痉挛的临床研究

摘　　要：目的观察川芎嗪联合尼莫地平防治破裂颅内动脉瘤栓塞术后脑血管痉挛的临床效果及安全性。方法回顾性分析2012-01—2012-06武警总医院神经血管外科介入栓塞的35例颅内破裂动脉瘤患者。根据术后采用川芎嗪联合尼莫地平或单独采用尼莫地平防治脑血管痉挛分为联合治疗组和对照组。联合治疗组20例,对照组15例,观察血管痉挛、迟发脑梗死以及预后情况。结果 2组脑血管痉挛发生率及迟发脑梗死差异有统计学意义(P<0.05),联合治疗组3个月预后良好患者明显高于对照组。结论川芎嗪联合尼莫地平防治破裂颅内动脉瘤栓塞术后脑血管痉挛的临床效果显著,不良反应小,可作为预防破裂颅内动脉瘤栓塞术后脑血管痉挛的联合方案。Objective To explore the clinical effect and safety of tetramethylpyrazine combined with nimodipin on prevention of cerebral vasospasm after embolization of ruptured aneurysms .Methods Thirty-five cases with ruptured aneurysms accepting endovascular embolization treatment from January 2012 to June 2012 were divided into two groups ,the control group was treated with Nimodipine for prevention of cerebral vasospasm ,and the combination treatment group was treated with tetra-methylpyrazine combined with nimodipin .The clinical efficacy and prognosis were compared .Results Significant differences between two groups were found in the ceveral vasoapasm and delayed cerebral infarction .During follow-up period ,combination group had better prognostic results than that in the control group （P〈0 .05） .Conclusion Tetramethylpyrazine combined with nimodipin is remarkably effective in the prevention of cerebral vasospasm after embolization of ruptured aneurysms ,which also has few adverse reactions and can be used as a combination therapy for cerebral vasospasm after embolization of intracranial ruptured aneurysms .
