摘 要:目的该研究主要就乙肝并发小肝癌患者的发展规律以及超声检查的临床应用价值展开分析讨论。方法选择该院2008年1月—2011年4月所收治的122例慢性乙肝患者作为研究对象,所有患者均是经过实验室检查,其结果显示患者的e抗原(HBeAg)、核心抗体(HBcAb)、表面抗原(HBsAg)均为阳性,所有患者在入院后均行二维超声检查以及三维图像重建,对其图像特点进行分析。并对患者肝内结节的二维以及三维超声检出率进行比较。结果在该研究的所有患者中,有34例患者出现了并发小肝癌的情况,对此类患者行三维超声检查可以对肝内的增生结节、小肝癌的形状及其与周边组织之间的关系进行清晰地显示,其检查出的小肝癌数量也明显多于二维超声的检出率,差异有统计学意义(Ρ<0.05)。结论在对乙肝患者进行临床诊断时,超声动态观察可以尽早发现患者肝内结节性病变,三维超声多角度、多切面的检查可以在一定程度上提高小肝癌的诊断准确性,因此,在对乙肝患者进行临床诊断时,可以对超声诊断的方式进行大力推广并普及使用。Objective This paper mainly analyzes and discusses the development law of hepatitis B complicated by small hepatocellular carcinoma and the clinical application value of ultrasound examination. Methods 122 cases of chronic hepatitis B patients admitted in our hospital from January, 2008 to April, 2011 were selected as the subjects. All the patients underwent laboratory examination, the results showed that the e antigen (HBeAg), core antibody (HBcAb), surface antigen (HBsAg) of them were positive. All patients underwent two -dimensional ultrasonography and three -dimensional ultrasonography image reconstruction after admission, the image characteristics were analyzed. And the detection rate of intrahepatic nodules between two-dimensional ultrasonography and three-dimensional ultrasonography was compared. Results Of all the patients in the study, 34 patients were found to have concurrent small hepatocellular carcinoma, and three-dimensional ultrasonography was performed in these patients, by which the nodular hyperplasia, the shape of small hepatocellular carcinoma and its relationship with the surrounding tissues were cleared showed. The detection rate for the number of small hepatocellular carcinoma was obviously higher than that of two-dimensional ultrasonography, the difference was statistically significant, Ρ&lt;0.05. Conclusion In the clinical diagnosis of hepatitis B patients, ultrasound dynamic observation can find intrahepatic nodules as soon as possible. 3D ultrasound examination can improve the diagnostic accuracy of small hepatocellular carcinoma multi-anglely and multifacetedly to a certain extent. Therefore, in clinical diagnosis of hepatitis B patients, ultrasonic diagnosis can be vigorously promoted and popularized.
