髂骨血管化移植与非血管化移植修复下颌骨缺损的比较

摘　　要：目的 :对髂骨修复下颌骨缺损的血管化与非血管化骨移植两种形式进行比较。方法 :对 71例连续行下颌骨切除和髂骨移植的病例进行了回顾性研究 ,对血管化与非血管化移植病例在手术过程及术后效果方面作了比较和分析。结果 :非血管化骨操作相对简单 ,用时较少 ,并发症发生率也低 ,但术后植入骨吸收明显 ,高度下降较大 ;血管化骨手术过程复杂 ,用时较长 ,并发症略高 ,但失败率低 ,植入骨吸收较少 ,高度下降不明显。结论 :血管化骨较非血管化骨有着无可比拟的优越性 ,但非血管化骨操作简单 ,目前两种形式均有其存在价值 ,适用于不同情况的病例。Objective:To compare the usage of vascularized and nonvascularized iliac graft for mandibular reconstruction.Methods:A retrospective study was made on 71 consecutive cases who were treated with mandibular segmental dissection and iliac graft reconstruction,the complications and effect of the two groups were compared.Results:The surgical procedure of nonvascularized iliac graft was relatively easy and complications were fewer,but the bone graft underwent substantial resorption in the later phase.The procedure of vascularized bone graft was complex and complications were a little more than that in nonvascularized bone graft.But the bone graft underwent little resorption.Conclusion:Vascularized iliac graft is more advantageous than nonvascularized iliac graft.Nonvascularized iliac graft is relatively simple and remains a usable choice in mandible reconstruction.
