摘　　要：目的探讨晶状体摘出联合玻璃体切除治疗晶状体脱位的术后效果及并发症的处理。方法对34例(34眼)晶状体脱位患者分别行晶状体摘出联合玻璃体切除手术(观察组)和单纯行晶状体摘出手术(对照组),观察2组患者的术后视力及并发症,分析两种术式的临床效果,采用t检验方法,以P<0.05为差异有统计学意义。结果34例晶状体脱位患者中,18例(18眼)采用晶状体摘出联合玻璃体切除手术,患者术后6个月视力提高者15例,未提高者3例,其中11例术后葡萄膜炎反应经双氯芬酸钠眼水、典必殊眼水及托吡卡胺眼水等治疗后10 d左右症状消失,5例眼压升高者经(0.2%卡替洛尔眼水)治疗2周左右眼压控制在正常范围,2例视网膜脱离患者经再次手术,并术后随访半年,未见并发症发生;另外16例(16眼)单纯行晶状体摘出手术,患者术后6个月矫正视力提高者6例,未提高者10例,其中6例发生视网膜脱离行玻璃体切除术视力未能提高,6例眼压升高患者经药物治疗未能好转,经小梁切除术仍有2例未能控制眼压,4例术后葡萄膜炎反应经双氯芬酸钠眼水、典必殊眼水及托吡卡胺眼水治疗后2周左右症状消失,2组术后视力恢复效果经统计学比较,差异有统计学意义(P<0.05)。结论晶状体摘出联合玻璃体切除治疗晶状体脱位,更有利于患者术后视力恢复及减少并发症的发生。Objective To investigate the outcome and complications in the treatment of lens dislocation by using lens ex- traction combined with vitreetomy. Methods 34 cases（ 34 eyes）of lens dislocation were enrolled in this study. The lens extraction combined with vitrectomy were performed in 18 cases （observation group）, while the lens extraction was per- formed in 16 cases （control group）. The visual acuity of patients, incidence of complications, and the clinical curative effect were compared between the two groups. T-test was employed to analyze the data, and the result will be statistically significant if the p-value is less than 0.05. Results In the observation group, the visual acuity of 15 patients were im- proved ,3 patients not improved within 6 months after the operation; 11 cases were with postoperative uveitis and the symp- toms disappeared in 10 days after using Diclofenac sodium eye water,TobraDex eye water and tropicamide eye drops ;5 ca- ses were with increased intraocular pressure and improved in 2 weeks by using O. 2% carteolol eye drops ;2 cases were with retinal detachment received the re-operation, the postoperative follow-up of six months did not find the complications. In the control group, the visual acuity of 6 patients were improved, 10 patients not improved within 6 months after the oper- ation;4 cases were with postoperative uveitis and the symptoms disappeared in 2 weeks after using Diclofenac sodium eye water,TobraDex eye water and tropicamide eye drops ;6 cases were with retinal detachment, and vitreous resection failing to improve their visual acuity;6 patients were with increased intraocular pressure ,all failed by medical treatment ,and the followed trabeculectomy was effective only 4 cases. The difference in the recovery of postoperative visual acuity between the two groups was statistically significant, P 〈 0.05.
