摘  要:目的探讨病情严重程度不同的阿尔兹海默病(AD)患者的营养状况及与AD的相关性。方法回顾分析2011年1月~2013年6月上海中医药大学附属普陀医院收治的96例AD患者的营养状况,并与同期100例健康体检者进行比较。营养状况的调查采用微型营养评定法(MNA)及生化指标的测定。根据简易智能状态量表(MMSE)将AD患者分为轻、中、重度3组,分析病情严重程度不同的AD患者营养状况的差异。采用Spearman相关分析及多元回归分析营养指标与AD的相关性。结果AD患者营养不良的发生率高于健康体检者,MNA评分、白蛋白(ALB)低于健康体检者(P〈0.05)。中度、重度AD患者营养不良的发生率高于轻度患者,ALB低于轻度患者;重度AD患者总蛋白低于轻度、中度患者,差异有统计学意义(P〈0.05)。Spearman相关分析表明,AD患者的ALB及MNA评分与MMSE评分呈正相关(P〈0.05);ALB与日常生活能力量表(ADL)评分呈负相关(P〈0.05)。多元回归分析表明,AD患者的MNA评分降低与MMSE评分、ADL评分呈线性相关(P〈0.05)。结论营养不良与AD疾病的发生发展密切相关。Objective To investigate the nutritional status of patients with different degree of Alzheimer% disease (AD) and the correlation between the nutritional status and AD. Methods The nutritional status of 96 patients with AD in Putuo Hospital Affiliated to Shanghai University of Traditional and Chinese Medicine from January 2011 to June 2013 were retrospectively analyzed and compared with 100 healthy controls. Mini-nutritional assessment (MNA) and measurement of biochemical indexes were used to investigate the nutritional status. According to the mini-mental state examination (MMSE), patients with AD were divided into mild, moderate, severe groups, and the difference of the nutrition status between different degree of AD was analyzed. Spearman correlation analysis and multiple regression analysis were used to study the correlation between the nutrition indexes and AD. Results Incidence rate of malnutrition in patients with AD was higher than that in healthy controls, MNA and albumin were lower than those in healthy controls, the differ- ences were statisticaly significant (P 〈 0.05). Incidence rate of malnutrition in moderate and severe groups was higher than that in mild group and albumin was lower than that in mild group, while the total protein in severe group was low- er than that in mild group and moderate group (P 〈 0.05). Spearman correlation analysis showed that ALB and MNA were positively correlated with MMSE, ALB was negatively correlated with ADL (P 〈 0.05). Multiple regression analysis showed that MNA lower degree was linearly correlated with MMSE and ADL (P 〈 0.05). Conclusion Malnutrition is closely related to the occurrence and development of AD.
