骨髓增生异常综合征对白血病的诊断价值分析

摘　　要：目的探讨骨髓增生异常综合征对白血病的诊断价值。方法选取2010年2月至2013年9月间收治的急性髓系白血病患者120例作为观察组,选择同期进行健康体检者120例作为对照组,两组均进行血常规检测与细胞形态学分析,同时抽取骨髓进行骨髓免疫检测和骨髓增生程度判定。结果观察组患者的白细胞、红细胞与血小板计数均明显低于对照组,差异有统计学意义(P<0.05)。观察组患者的细胞形态学主要表现为细胞大小明显不均匀,核形态分叶过多、形态异常,染色质网状排列等。观察组患者的CD+34CD+123/CD+34比例为(38.24±14.74)%,而对照组为(1.90±0.89)%,差异有统计学意义(P<O.05)。观察组患者多表现为骨髓增生减低或极度减低,而对照组多为骨髓增生活跃或明显活跃,差异有统计学意义(P<0.05)。结论骨髓增生异常综合征对白血病的诊断有很好的参考价值,而检测白血病干细胞的含量有助于评价患者的疾病程度与预后。Objective To investigate the diagnostic values of myelodysplastic syndrome for the leukemia patients. Methods Selected 120 acute myeloid leukemia patients from February 2010 to September 2013 in our hospital as the observation group, and selected 120 healthy people at the same period in our hospital as the control group, two groups were given the blood tested and analyzed of cell morphology, and selected bone marrow for bone marrow hyperplasia immunoassay determination. Results The WBC, RBC and platelet counts in the observation group were significantly less than the control group, the difference were statistically significant （P 〈 0. 05）. the morphological showed were mainly for cell size were significantly uneven, excessive leaf nuclear morphology, morphological abnormalities, chromatin mesh arrangement of the observation group, the CD3^＋ CD123 ^＋/ CD34^＋ values of the observation group was（38.24 ± 14. 74）%, while the control group was （ 1.90 ± 0. 89） %, the difference was statistically significant （P 〈 0. 05 ）. While the patients of the observation group were showed more extreme myeloproliferative reduce or minimize, so that were showed yeloproliferative or apparent actively in the control group which the difference were statisti- cally significant（P 〈 0. 05）. Conclusion The diagnosis of myelodysplastic syndrome leukemia has better reference value, and to detect the content of leukemia stem cells is contribute to the extent of disease and prognostic evaluation of patients.
