内眼手术前两种皮肤准备方法的对比研究

摘　　要：目的探讨内眼手术前不剪除睫毛备皮方法的可行性及价值。方法选取2012年12月至2013年12月眼科择期行内眼手术的患者200例,随机分为实验组和对照组。实验组100例,手术前不剪除睫毛;对照组100例,术前按照传统的皮肤准备方法剪除睫毛,其他术前准备两组均相同。观察比较两组患者术前冲洗术眼结膜囊后、术前消毒术眼后即刻、手术完毕即刻3个时间点结膜囊标本细菌培养阳性率,术后两组患者眼部刺激症状、并发症及操作的便利性等。结果两组患者结膜囊3个时间点细菌培养结果的阳性率差异无统计学意义(P>0.05);对照组手术后眼部刺激症状比实验组明显且持续时间长(P<0.05);实验组术前准备需要的人力和物力成本较低。结论内眼手术前皮肤准备的关键是彻底清洁眼睑及周围皮肤、结膜囊冲洗消毒。术前不剪睫毛不会增加术后细菌感染的风险,且减轻术后眼部的不适症状,提高护士工作效率。Objective Discussed in the feasibility and value of skin preparation methods to not cut off eyelashes before internal ophthalmologic operation. Methods Selected 200 cases of patients who will undergo elective internal ophthalmologic operation from December 2012 to December 2013 and randomly divided into experimental group and control group. 100 cases in experimental group were not cut off the eyelashes before operation and 100 cases in control group were cut off eyelash in accordance with the traditional method of skin preparation. Others are the same peroperative preparation process. Compared the positive rate of bacterial culture of conjunctival sac specimen at three time points which are after flushing technique of conjunctiva sac before surgery, preoperative disinfection eye immediately before the operation and after the operation immediately, and the eye irritation, complications and convenience of operation and so on in two groups of patients. Results The differences between two groups of patients about positive rate of bacterial culture of conjunctival sac speciment had no statistical significance （P〉 0. 05） at three time points. However, the eye irritation of control group after operation is more obvious and last longer time （P〈0. 05） than experimental group. Conclusion Skin preparation method before internal ophthalmologic operation is the key to thoroughly clean the eyelids and surrounding skin and conjunctival sac flushing disinfection. It will not increase the risk of postoperative bacterial infection and reduce the postoperative symptoms of eye and improve the nurse’s work efficiency, which is not cut eyelash before internal ophthalmologic operation.
