济泰片与美沙酮治疗社区戒毒人员抑郁症状的疗效比较

摘　　要：目的比较济泰片与美沙酮治疗社区戒毒人员抑郁症状的疗效。方法采用为期2年的开放性队列对照研究设计,将上海市7个区65个街道符合入组标准的386名社区戒毒人员分为济泰片组(206名)和美沙酮组(180名)。济泰片组给予济泰片口服治疗1年,停药后继续观察1年;美沙酮组给予美沙酮维持治疗2年,且在第一年内对两组同时合并实施团体心理咨询和社会支持干预。采用抑郁自评量表(SDS)分别在7个随访时点,对其抑郁症状进行测评。结果在改善抑郁症状方面,从基线到104周末,济泰片组SDS标准评分的中位数从50分下降到43分,美沙酮组从51分下降到49分;经混合效应模型检验及多重比较,两组抑郁标准评分在时间和组别两个效应上的差异均有统计学意义(P<0.05),并且时间效应与组别效应之间有交互作用(P<0.05),即反映两组的SDS标准评分随时间变化趋势有所不同,济泰片组SDS标准评分较美沙酮组下降更多。结论在社区戒毒康复中,济泰片与美沙酮治疗合并团体心理咨询和社会支持干预,均能改善社区戒毒人员的抑郁症状,但济泰片的效果略优于美沙酮。OBJECTIVE To compare the effectiveness of Jitai tablets versus methadone in community depression treatment. METHODS A 2-year cohort study was conducted, 386 eligible subjects from 7 districts and 65 communities in Shanghai entered the cohort, Jitai tablets group（n=206） and methadone group（n=180）. The Jitai tablets group was treated by Jitai tablets for 1 year, after then was observed for another 1 year. The methadone group was treated by methadone for 2 years. Both groups were intervened with psychological intervention and social support in the first year during the treatment. The degree of depression was measured by Self-Rating Depression Scale（SDS）at seven follow-up time points respectively. RESULTS From baseline to 104 weekend, the median of Jitai tablets group＇s SDS scores descended from 50 to 43 while the methadone group from 51 to 49. Mixed model test indicated significant difference in group effect, time effect and interaction effect（P〈0.05） , That means both groups＇ SDS scores had been trending downward from baseline to 104 weekend, and the Jitai tablets group＇s SDS scores had fallen slightly more than the methadone group. CONCLUSIONS Methadone and Jitai tablets were combined for psychological intervention and social support both gained effective depression treatment in community, but Jitai tablets had better efficacy.
