摘  要:目的探讨骨样骨瘤的多层螺旋CT(MSCT)影像学表现及对该病的诊断价值。方法回顾性分析经病理证实的87例骨样骨瘤的临床和影像学资料,按检查方法分为MSCT组33例及X线平片组54例,比较MSCT和普通X线摄影术诊断本病的优势与不足。结果本组男57例,女30例;年龄5-38(19±5.7)岁。MSCT组、X线平片组诊断微小病灶(直径〈5 mm)骨样骨瘤分别为15例(45.5%),3例(5.6%);诊断骨质钙化分别为33例(100%),49例(90.7%),组间比较差异均有统计学意义(P〈0.05)。MSCT组及X线平片组检出骨皮质型骨样骨瘤分别为18例(54.5%),11例(20.4%),关节型骨样骨瘤分别为2例(6.1%),16例(29.6%),组间比较差异有统计学意义(P〈0.05)。结论 MSCT诊断微小骨样骨瘤、瘤巢内骨质钙化灶的准确率及敏感性均较高,而X线平片对关节型骨样骨瘤的诊断敏感性较高。Objective To explore the imaging features and diagnostic value of osteoid osteomas( OOs) in multislice CT( MSCT) examination. Methods Clinical and imaging data of 87 OOs patients confirmed by pathology was retrospectively analyzed,and the patients were divided into MSCT group( n = 33) and X-ray group( n = 54) according to different examination methods,and then the advantages and disadvantages of MSCT and ordinary X-ray photography in diagnosis value were compared. Results There were 57 males and 30 females of 5- 38 years old( average 19 ± 5. 7). Micro-lesions OOs( diameter 5 mm) were found in 15 cases( 45.5%) in MSCT group and 3 cases( 5.6%) in X-ray group; sclerotin calcification was found in 33 cases( 100%) in MSCT group and 49 cases( 90. 7%) in X-ray group,and the differences in the two groups were statistically significant( P 〈0. 05). Cortical bone OOs and articular OOs were detected at 54. 5% and 6. 1% in MSCT group and 20. 4% and 29. 6% in X-ray group respectively,and the differences in the two groups were statistically significant( P 〈0. 05). Conclusion MSCT in diagnosis of micro-OOs and sclerotin calcification lesions is more sensitive and accurate,while X-ray plain film is more sensitive in diagnosis of articular OOs.
