摘 要:目的 在晚期非小细胞肺癌(NSCLC)患者中,^18F-FDG PET/CT的最大标准化摄取值(SUVmax)已被证实是独立的预后因子,本研究旨在探讨SUVmax对NSCLC患者术后复发的预测作用.方法 回顾性分析了本中心NSCLC手术患者资料.随访至2014年1月31日.x2检验分析SUVmax与临床特征之间的相关性,Kaplan-Meier分析及COX比例风险模型分析SUVmax与术后无病生存期(DFS)及总生存期(OS)的关系.结果 研究共纳入79例NSCLC手术患者,中位生存时间为35.1个月(8.1~77.2个月).以全部研究人群的原发病灶SUVmax中位值8.4为界,SUVmax<8.4组的患者术后中位DFS及中位OS均大于SUVmax≥8.4组(P值分别为0.000、0.001).在多因素回归分析中,SUVmax与DFS及OS均有明显的正相关性(P值分别为0.002和0.029).结论 ^18F-FDG PET/CT的SUVmax可用于评价NSCLC患者术后复发和预后.相对于SUVmax值低的患者,SUVmax值高的患者复发的概率相对高,且预后较差.Objective Maximal standardized uptake value (SUVmax) of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (^18F-FDG PET/CT) has been shown to be a prognostic indicator for advanced non-small cell lung cancer (NSCLC) patients.This study aimed to investigate the predicting value of SUVmax in recurrence and prognosis of early NSCLC patients with complete resection.Methods NSCLC patients who were examined by 18F-FDG PET/CT before surgery and took complete resection were retrospectively reviewed.All the clinical information was collected.The association between pretreatment SUVmax and clinical characteristic was analyzed using chi-square test.The correlation between SUVmax and disease free survival and overall survival was analyzed using KaplanMeier survival analysis and COX proportional hazards model.Results Seventy-nine NSCLC patients with both 18F-FDG PET/CT examination and complete resection were enrolled into this study.Median overall survival was 35.1 months (8.1-77.2 months).The median value of SUVmax in total patients was 8.4.The madian disease free survival and overall survival in patients with SUVmax〈8.4 were longer than those in patients with SUVmax ≥ 8.4 (P =0.000,0.001,respectively).In the multivariate analysis,SUVmax was positively correlated with disease free survival and overall survival (P =0.002,0.029,respectively).Conclusions SUVmax of ^18F-FDG PET/CT is an independent predictive factor for recurrence and prognosis in NSCLC patients.High SUVmax is associated with high recurrence rate and poor prognosis in NSCLC patients with complete resection.
