摘 要:目的通过分析不同龈沟深度的种植体周围菌群分布的特点,了解龈沟深度与菌群定植的关系。方法选择我院口腔科进行义齿种植的患者38名作为研究对象,测量患者种植体周围袋深度(Peri-implant Probing pokect Depth,PPD),并根据PPD将研究对象划分为一组(PPD<3 mm)及二组(PPD≥3 mm)两组,比较两组患者改良菌斑指数、牙龈出血指数种植体周围菌群的检出率及CFU构成情况。结果本次共检查了78颗种植体,一组55颗种植牙、二组共23颗种植牙,一组患者的mPLI平均为0.67±0.71、GBI平均为0.43±0.54;二组患者的mPLI平均为1.74±0.89、GBI平均为1.67±0.68。一组及二组调查对象种植体的mPLI及GBI差异具有统计学意义(P<0.001)。有益菌血链球菌及小韦荣菌一组检出率高于二组(P<0.05);而致病菌具核梭杆菌、中间型普氏菌检出率二组高于一组(P<0.05),变黑普雷沃菌、牙龈卟啉菌在两组间的检出率无差异。一组与二组各类菌群菌落形成单位的构成比总体分布差异具有统计学意义(P<0.001),一组患者有益菌群革兰阳性兼性厌氧菌如血链球菌及其与链球菌的比例较高,而有害菌群革兰氏阴性厌氧杆菌如具核梭杆菌、变黑普雷沃氏菌所占比例较低,而二组则呈相反的分布状况。结论种植体周龈沟深度与菌群定植有一定的关系,龈沟较深的种植体周龈下菌群有益菌较少,而有害菌所占比例较高。Objective To analyze the characteristics of flora distribution around implant with different gingival sulcus depths and to learn the relationship between gingival sulcus depth and microbes colonization. Methods 38 patients with implant treatment in our hospital were chosen as the research objects, and the peri-implant probing pocket depths(PPD) were detected, and according to the PPD, the patients could be divided into the group 1 with PPD3 mm) and the group 2 with PPD ≥ 3 mm. Improved plaque index,gingival bleeding index and the detection rate of implant surrounding flora and CFU were compared between the 2 groups. Results78 implants were examined, with 55 implants in the group 1 and 23 implants in the group 2. In the group 1, PLI was averagely 0.67+ /- 0.71, and GBI was averagely 0.43 + /- 0.54; in the group 2, PLI was averagely 1.74 + /- 0.89, and GBI was averagely 1.67 +/- 0.68. There were significant differences of PLI and GBI between the 2 groups(P0.001). Streptococcus sanguis and Veillonella parvula in the group 1 were higher than that of the group 2(P〈0.05); Fusobacterium nucleatum and Prevotella intermedia in the group 2 were higher than that in the group 1(P〈0.05), Prevotella nigrescens and Porphyromonas gingivalis had no differences between the two groups. The distribution of overall constituent ratio of every flora had statistical differences between the 2 groups(P〈0.001). In the group 1, the ratio of gram-positive facultative anaerobe bacteria such as Streptococcus sanguis and other streptococcus were higher, and the ratio of gram-negative anaerobic bacteria such as Fusobacterium nucleatum and Prevotella nigrescens were lower, but the ratio distribution was totally opposite in the group 2. Conclusion Gingival sulcus depth around implant is related with microbes colonization. In the deep sulcus, the proportion of beneficial bacteria is lower and the proportion of harmful bacteria is higher.
