离体猪食管曲张静脉压力对套扎效果影响的实验研究

摘　　要：目的探讨离体猪食管曲张静脉压力对多环套扎器完全套扎效果的影响。方法选取实验用猪的静脉血管制成离体不同静脉压力的静脉曲张血管压力模型，按预设压力范围分成3组，P1组25—30cmH2O（1cmH2O=0．098KPa）、P2组35～40cmH2O、P3组45～50cmH2O，然后制成离体猪食管静脉曲张模型，统计分析各组体外套扎效果。结果Pl组完全套扎75个（47．77％，75／157），P2组完全套扎41个（32．28％，41／127），P3组完全套扎29个（23．58％，29／123），各组间完全套扎率比较差异有统计学意义（χ2=19．5585，P=0．0006）。结论曲张静脉压力越高，套扎效果越不理想。测量曲张静脉压力可以预测内镜下套扎治疗效果，有助于指导内镜下治疗方案的选择。Objective To explore the influence of porcine esophageal variceal pressure on complete ligation for polycyclie ligator in vitro. Methods The experimental porcine venous vessels were selected to make the models of different venous pressure, which were divided into 3 groups according to the preset pres- sure range, P1 group 25-30 emH20（ 1 cmH20 = 0. 098 KPa）, P2 group 35-40 cmHzO, P3 group 45- 50 cmH20. Ligation effects of each group were analysed. Results There were 75 complete ligations （47.77% ,75/157） in group P1, 41 in group P2 （32.28% ,41/127）, and 29 in group P3 （23.58%, 29/123）. There were significant differences among the three groups （χ2 = 19. 558 5, P =0. 000 6）. Con- dusion The higher the variceal pressure is, the worse the effect of ligation is. Variceal pressure could pre- dict the effect of endoscopic ligation, and is helpful for the choice of endoscopic treatment.
