摘　　要：目的观察弹簧圈联合Onyx18栓塞大型颈内动脉侧壁动脉瘤的疗效，探讨其栓塞技术及疗效。方法回顾分析收治的大型颈内动脉侧壁动脉瘤患者10例（动脉瘤10个）临床资料，所有动脉瘤均采用弹簧圈联合Onyx18栓塞，评价动脉瘤栓塞程度，围术期并发症及预后。结果术中即刻动脉瘤完全栓塞7例，瘤颈残留2例，瘤体残留1例，术后继发短暂性神经功能缺损2例，其中过度栓塞1例；随访时间16个月，恢复良好10例；DSA复查8例，动脉瘤完全闭塞5例，再通1例，未变化1例，载瘤动脉闭塞1例。结论弹簧圈联合Onyx18可用于大型颈内动脉侧壁动脉瘤的治疗。Objective To investigate the safety of combination of coils and Onyx 18 for the treatment of large sidewall aneurysms of internal carotid artery. Methods Ten patients with ten large sidewall aneurysms of internal carotid artery were treated with combination of coils and Onyx 18, and the clinical data were recorded. The immediate occlusion of aneurysms, complications and outcomes were reviewed. Results Seven aneurysms were completely occluded. Two aneurysms with neck remnants and one with dome remnant were shown at the immediate angiography. Two patients had transient neurological deficits. All of the patients were followed - up with 16 months of median duration. Good outcomes were achieved in all the patients without neurological deficits. Eight patients received DSA examination during the follow -up period. One aneurysm was recanalization, and one remained stable. There was no recanalization in five aneurysms, but one patient had the occluded parent artery. Conclusions Combination of coils and Onyx 18 could be used for the treatment of large sidewall aneurysms of internal carotid artery.
