摘  要:目的判断磁共振弥散张量成像(DTI)在急性脑梗死患者运动功能预后的临床价值。方法对30例急性脑梗死患者行磁共振(MRI)和DTI影像学检查,检测参数为部分各向异性值(FA),同时对脑梗死患者进行Brunnstrom评分。结果脑梗死患者患侧白质FA值显著低于健侧(P<0.01),统计学分析结果显示患侧皮质脊髓束损伤程度与偏瘫侧肌力明显相关(Y=0.774,P<0.05),30例患者DTI分级与肌力减弱程度呈正相关(rs=0.75,P<0.05)。结论 DTI技术能清楚显示脑梗死白质纤维束的受累程度,能够较好的评价患者运动功能损伤,对急性脑梗死患者运动功能预后判断有重要临床价值。Objective To determine the clinical value of magnetic resonance diffusion tensor imaging in prognosis of motor function in patients with acute cerebral infarction .Methods 30 cases of acute cerebral infarction were accepted by magnetic resonance imaging (MRI) and magnetic resonance diffusion tensor imaging (DTI),with fractional anisotropy ( FA ) as the detection parameter .Additionally , Brunnstrom scores were also uesd for these patients.Results FA of white matters in affected side of the patients with cerebral infarction was significantly lower-than the contralateral side ( P〈0.01) .Statistical analysis showed that the degree of ipsilateral corticospinal tract injury was significantly associated with muscle strength of hemiplegic side (Y=0.774,P〈0.05).The DTI grading of these 30 patients was positively correlated with the degree of their muscle weakness ( rs=0.75, P〈0.05) .Conclusion DTI can demonstrate the degrees of involvement of the fiber bundles in white matters of patients with cerebral infarction clearly, well evaluating the motor dysfunction .It has important clinical value in predicting the prognosis of motor function in patients with acute cerebral infarction .
