活动期溃疡性结肠炎患者血清vaspin水平及其临床意义

摘　　要：背景：脂肪细胞因子与机体内能量稳定相关，并介导多种免疫应答和炎症反应。内脏脂肪特异性丝氨酸蛋白酶抑制剂 vaspin 是近年发现的一种与炎症反应有关的脂肪细胞因子。目的：检测活动期溃疡性结肠炎（UC）患者的血清 vaspin 水平并探讨其临床意义。方法：选取2008年1月～2013年4月苏州市立医院收治的150例活动期UC 患者，以150名健康体检者作为正常对照组。采用 ELISA 法检测血清 vaspin 水平，并分析其与 UC 临床特征的相关性。结果：UC 患者血清 vaspin 水平显著高于正常对照者[（1．86±0．38）μg/L 对（0．96±0．43）μg/L，P ＜0．01]，并与血清 CRP 水平和疾病活动指数呈显著正相关（r ＝0．628，P ＜0．01；r ＝0．514，P ＜0．05），与血清 ESR水平和病变部位无关（r ＝0．098，P ＞0．05；r ＝0．124，P ＞0．05）。结论：Vaspin 可能在 UC 发生、发展的病理生理机制中发挥重要作用。Background：Adipocytokines are associated with energy homeostasis and mediate various immune responses and inflammatory reactions.Vaspin is a novel adipocytokine that is thought to be associated with inflammatory reaction.Aims：To investigate the serum vaspin level and its clinical significance in patients with active ulcerative colitis （UC）.Methods：Serum vaspin level was determined by ELISA in 150 patients with active UC and 150 healthy controls in Suzhou Municipal Hospital from January 2008 to April 2013,and the correlation of serum vaspin level with the clinical characteristics of UC was analyzed.Results：Serum vaspin level in patients with UC was significantly higher than that in normal controls [（1.86 ±0.38）μg/L vs.（0.96 ±0.43）μg/L,P 〈0.01].There was significant positive correlation between serum vaspin level and serum CRP and disease activity index in UC patients （r =0.628,P 〈0.01;r =0.514,P 〈0.05）,but serum vaspin level was not correlated with serum ESR and location of UC （r =0.098,P 〉0.05;r =0.124,P 〉0.05）.Conclusions：Vaspin may play an important role in the pathophysiology of UC.
