摘 要:目的通过建立移植肝胆管损伤模型,探讨超音速剪切波弹性成像评价移植肝冷保存损伤后胆管再生能力的价值。材料与方法 96只成年雄性大鼠随机分为两组,实验组48只,24只供体,24只受体,供肝4℃保存12 h后移植;对照组48只,24只供体,24只受体,常规肝移植。每组大鼠随机分为3个亚组,分别于术后1周、2周、4周进行超声弹性成像检查,并取肝脏标本行病理检查,比较各组肝脏弹性指数及实验组各亚组的门管区平均胆管数量。结果实验组大鼠肝移植术后1周、2周、4周较对照组肝脏弹性指数明显升高(F=89.98,P<0.01),实验组1周与2周、2周与4周组间肝脏弹性指数差异有统计学意义(P<0.05)。实验组术后2周、4周时肝脏弹性指数与肝脏门管区胆管增生数量呈显著正相关(r=0.898,P<0.01)。结论肝移植术后肝脏弹性指数的改变与胆管增生数量显著相关,在评估肝移植术后胆管增生方面具有一定的应用价值。Purpose To explore the value of supersonic shear wave elastography in assessing biliary regeneration of liver transplantation under cold preservation by using biliary duct injury model. Materials and Methods Ninety-six SD rats were randomly equally divided into experimental group and control group, each including 24 donors and 24 recipients. The donor livers in the experimental group were stored at 4 ℃ for 12 hours and then performed the transplantation; while those in the control group were performed the transplantation in the conventional way. The animals in each group were further randomly divided into three subgroups, and underwent supersonic shear wave elastography at 1 week, 2 weeks and 4 weeks aRer operations. The liver specimens were examined pathologically. The liver stiffness index and average number of bile ductules in the portal area were compared among the subgroups. Rosults The liver stiffness index in the experimental group was significantly higher than that in the control group at 1 week, 2 weeks and 4 weeks after operation (F=89.98, P〈0.01). In the experimental group, the differences were significant either between 1 week and 2 weeks or between 2 weeks and 4 weeks (P〈0.05). Moreover, the liver stiffness index was positively correlated with the number of biliary proliferation in the subgroups of experimental group at 2 weeks and 4 weeks after operation (r=0.898, P〈0.01). Conolusion The change of liver stiffness is 'significantly correlated with the number of biliary proliferation after liver transplantation, which shows that shear wave elastography is valuable in the assessment of biliary proliferation.
