B超联合CT下行射频消融术治疗原发性肝癌的临床效果观察

摘　　要：目的：探讨在 B 超联合 CT 下和在 CT 下行射频消融术（RFA）治疗原发性肝癌的临床效果。方法原发性肝癌患者82例，均行经肝动脉化疗栓塞术治疗，术后1周41例患者在 B 超联合 CT 下行 RFA 治疗（治疗组），余41例在 CT 下行 RFA 治疗（对照组），观察两组患者的肿瘤体积缩小率和0.5、1、2年生存率。结果治疗组和对照组肿瘤体积缩小总有效率分别为95.12％（39/41）和80.49％（33/41），组间比较差异有统计学意义（χ^2=5.33，P ＜0.05）。治疗组治疗后0.5、1、2年生存率分别为100％（41/41）、82.9％（34/41）和73.2％（30/41）；对照组分别为100％（41/41）、70.7％（29/41）和51.2％（21/41），两组治疗1、2年生存率比较差异均有统计学意义（χ^2值分别为3.96、5.07，P 均＜0.05）。结论在 B 超联合 CT 下行 RFA 治疗原发性肝癌的临床效果明显优于 CT 下行 RFA 治疗组。Objective To explore the clinical effect of radio frequency catheter ablation（ RFCA） guiding by B ultrasound combined with CT on treating primary hepatic carcinoma. Methods Eighty-two patients with primary liver cancer were treated with transcatheter arterial chemoembolization therapy. One week after the operation,41 patients were treated with RFCA guiding by B ultrasound combined with RFA therapy and served as treatment group. Another 41 cases were given RFA treatment guiding by CT as control group. The indices including tumor shrinkage rate,survival rate of 0. 5,1 and 2 years. Results The tumor shrinkage and total effective rates in the treatment and control group were 95. 12%（39 / 41）and 80. 49%（33 / 41）,and there was statistically significant difference between the two groups（χ^2 = 5. 33,P 〈 0. 05）. After treatment,the survival rates of 0. 5,1and 2 years in treatment group were 100%（41 / 41）,82. 9%（34 / 41）and 73. 2%（30 / 41）,and that of the control group were 100%（41 / 41）,70. 7%（29 / 41）and 51. 2%（21 / 41）. There was significant differences between groups in terms of the survival rates of 1and 2 years（ χ^2 = 3. 96,5. 07,P 〈 0. 05）. Conclusion The curative effect of RFA guiding by B ultrasound combined with CT is significantly superior to that guiding by CT.
