摘 要:目的探讨腮腺切除术中面神经暴露长度与其功能损失程度之间的相关性。方法对年龄30-55岁、术前无面瘫的14例腮腺肿瘤患者行保留面神经的腮腺切除术,术中测量所暴露下颌缘支的长度,术后2周对患侧和健侧下颌缘支行神经电图(electroneurography,EN0G)检测,计算患侧EN0G波幅损失百分比及潜伏时延长百分比,对下颌缘支暴露长度、EN0G波幅损失百分比、潜伏时延长百分比三个变量行正态性检验,三变量总体均服从正态分布,分别绘制下颌缘支暴露长度与EN0G波幅损失百分比、潜伏时延长百分比双变量间的散点图,并分别行Pearson相关分析。结果下颌缘支暴露长度与EN0G波幅损失百分比之间的散点图有线性趋势(r=0.497,P=0.071)。下颌缘支暴露长度与EN0G潜伏时延长百分比之间的散点图无线性趋势(r=0.083,P=0.779)。结论腮腺切除术中,面神经的暴露长度与其功能损失程度之间尚不能认为有相关性,面神经的暴露长度可能通过其他因素影响面神经功能,不是影响面神经功能的主要因素或直接因素。Objective To explore the correlation between exposed length and degree of functional injury of the facial nerve in parotidectomy. Methods Fourteen cases of parotid tumor aged 30 -55 years, who had no facial paralysis before surgery, were enrolled in this study and underwent parotidectomy with facial nerve preservation. During the operation, the exposed length of the marginal mandibular branch of facial nerve was measured. Electroneurographic examination was carried out for the marginal mandibular branch of facial nerves on the affected sides and normal sides at 2 week after the operation. The percentages of reduction of evoked potential amplitude and the percentages of extension of latency on the affected sides were calculated according to the value of normal sides. Normal test was done respectively to study the distribution of the exposed length of the marginal mandibular branch of facial nerve, the percentages of reduction of evoked potential amplitude, and the percentages of extension of latency. All of them satisfied normal distribution. Then the scatter diagram and Pearson correlation analysis between exposed length of the marginal mandibular branch of facial nerve and the per- centages of reduction of evoked potential amplitude, and between exposed length of the marginal mandibular branch of facial nerve and the percentages of extension of latency were carried out respectively. Results The scatter diagram between exposed length of the mar- ginal mandibular branch of facial nerve and the percentages of reduction of evoked potential amplitude reflected the linear trend ( r = 0.497, P = 0.071 ). The scatter diagram between exposed length of the marginal mandibular branch of facial nerve and the percentages of extension of latency did not reflect the linear trend( r = 0.083,P = 0. 799). Conclusion There is no evidence that the correlation exists between exposed length and degree of functional injury of the facial nerve in parotidectomy.
