三维适形放疗联合化疗治疗局部晚期鼻咽癌的临床观察

摘　　要：目的探讨三维适形放疗联合同期化疗加辅助化疗治疗局部晚期鼻咽癌的治疗效果、毒副反应。方法 120例局部晚期鼻咽癌患者随机分为观察组和对照组,2组均采用三维适形放疗,观察组患者在放疗的基础上给予同期化疗和辅助化疗。结果观察组和对照组近期有效率分别为90.00%、70.00%,差异有统计学意义(P<0.05);3 a总生存率分别为81.67%、68.33%,差异有统计学意义(P<0.05);3 a无瘤生存率分别为71.67%、58.33%,差异有统计学意义(P<0.05)。观察组血液学毒性和胃肠道反应发生率较对照组高,差异有统计学意义(P<0.05)。结论三维适形放疗联合同期化疗加辅助化疗治疗局部晚期鼻咽癌能够有效控制肿瘤和改善生存状况。Objective To evaluate the efficacy and toxicities of three-dimensional conformal radiotherapy combined with concurrent chemotherapy plus adjuvant chemotherapy in the treatment of patients with locally advanced nasopha- ryngeal carcinoma. Methods One hundred and twenty patients with locally advanced nasopharyngeal carcinoma were randomly divided into the observation group and the control group. The patients of two groups were treated with three- dimensional confonmal radiotherapy, and the patients of the observation group were treated with concurrent chemothera- py plus adjuvant chemotherapy on the basis of radiotherapy. Results The efficacy rate was 90.00% of the observation group,and was 70.00% of the control group （P 〈 0.05 ）. The 3-year survival rate was 81.67% of the observation group, and was 68.33% of the control group（ P 〈 0.05 ）. The 3-year disease-free survival rates was 71.67% of the ob- servation group, and was 58.33% of the control group（ P 〈 0.05 ）. The hematologic toxicity and gastrointestinal reac- tion incidences of the observation group were higher than those of the control group （P 〈 0.05）. Conclusion Three- dimensional conformal radiotherapy combined with concurrent chemotherapy plus adjuvant chemotherapy in the treat- ment of patients with locally advanced nasopharyngeal carcinoma can control the tumor progression and improve the sur- vival.
