CT引导局部瘤内注药术治疗转移瘤的临床研究

摘　　要：目的评价局部瘤内注药术联合抗血管治疗控制转移瘤的安全性和有效性。方法 17例转移瘤患者均行CT引导下局部瘤内注药术,注射药物为顺铂、重组人血管内皮抑制素注射液及碘油。定期行CT或MRI复查,客观疗效评价参照RECIST实体肿瘤评价方法。结果 17例共进行60次治疗,平均每位患者行穿刺治疗3次。无因穿刺及注药引起的并发症。随访率100.0%,17例患者均存活。其中获CR 0例(0%),获PR 7例(41.2%),SD 9例(52.9%),PD1例(5.9%),RR 8例(41.2%),DCR 16例(94.1%)。临床症状均得到改善,其1年生存率为88.2%,2年生存率为47.2%,3年生存率为23.5%。结论 CT引导下顺铂联合重组人血管内皮抑制素注射液及碘油的局部瘤内注药术是控制转移瘤的一种安全、有效、微创的方法,可发展成为缓解晚期肿瘤患者临床症状的姑息治疗方案。Objective To evaluate the safety and effectiveness of local intratumoral injection combined with anti-angiogenesis therapy to control metastasis .Methods 17 cases of metastatic tumor were treated with local intratumoral injection under CT guidance .The injection of cisplatin combined with recombinant human endostatin and lipiodol was performed . The objective evaluations were according to regular CT or MRI and RECIST solid tumor evaluation method .Results There were 60 treatments in 17 parients and 3 times of puncture treatment per patient .No complications were caused by puncture and injection .The follow-up rate was 100% .All the 17 patients were alive .There were CR in 0 cases （0% ） , PR in 7 cases （41 .2% ） ,SD in 9 cases （52 .9% ） ,PD in 1 （5 .9% cases） ,RR in 8 cases （41 .2% ） ,DCR in 16 cases （94 . 1% ） .The clinical symptoms were improved and 1 year survival rate was 88 .2% ,2 year survival rate was 47 .2% ,3 year survival rate was 23 .5% .Conclusion The local intratumoral injection of cisplatin combined with recombinant human endostatin and iodized oil under CT guidonle is a safe ,effective ,minimally invasive method for controlling metastases .It can be a development of palliative care programs to relieve clinical symptoms in patients with advanced cancer .
