腰大池腹腔分流术与脑室腹腔分流术治疗特发性颅内压增高的疗效比较

摘　　要：目的：回顾分析2009年3月～2012年6月收住我院神经外科12例腰大池腹腔分流治疗特发性颅内压增高与12例脑室腹腔分流治疗特发性颅内压增高的临床资料。探讨腰大池腹腔分流与脑室腹腔分流治疗特发性颅内压增高症的手术疗效及并发症。方法术前常规测量视力、视野、眼底、激素水平，腰穿测量颅内压，并做MRI检查，腰大池腹腔分流术后视力与脑室腹腔分流术后视力比较，评价腰大池腹腔分流与脑室腹腔分流治疗特发性颅内压增高的临床疗效。结果腰大池腹腔分流病人组与脑室腹腔分流病人组视力、视野比较，两组差异有统计学意义（P＜0．05）。结论腰大池腹腔分流术组较脑室腹腔分流术组对特发性颅内压增高患者的视力恢复效果更佳。Objective To retrospectively analysis 12 cases of lumbar shunt treatment of idiopathic intracra-nial hypertension and 12 cases of ventricular shunt treatment of idiopathic intracranial hypertension from March 2009 to June 2012 in Beijing Tongren Hospital Neurosurgery ,and to explore the surgical therapeutic effects and complications . Methods Routine preoperative visual acuity measurements ,vision,fundus images,hormone levels and lumbar puncture intracranial pressure were measured ,and the clinical therapeutic effects were evaluated by comparing visual acuity before and after surgey.Results It showed the significant difference （P〈0.05）.Conclusion Lumbar peritoneal shunt group show better than ventricular-peritoneal shunt group for idiopathic intracranial hypertension .
