早期卵巢癌腹腔镜下全面分期手术的临床效果

摘　　要：目的探讨早期卵巢癌腹腔镜下全面分期手术的临床效果。方法回顾性分析37例行腹腔镜下全面分期手术早期卵巢癌患者(腹腔镜组)及同期行开腹确定行分期手术的42例患者(开腹组)临床资料,比较两组的临床效果。结果两组均顺利完成手术。腹腔镜组术中出血量、术后下床时间、术后排气时间和术后住院时间均少于开腹组(P<0.05),但手术时间长于开腹组(P<0.05)。术后随访6-36个月,两组患者术后并发症发生率、术后卵巢癌分期上升发生率、术后化疗率和化疗疗程以及卵巢癌复发率方面均无统计学差异(P>0.05)。结论早期卵巢癌行腹腔镜下全面分期手术的近期手术效果与开腹手术相近,但腹腔镜手术创伤小,术后恢复快,可作为早期卵巢癌全面分期手术治疗的术式。Objective To investigate the outcomes of comprehensive staging surgery under laparoscope for early-stage ovarian cancer. Methods Data of 79 patients with early-stage ovarian cancer underwent surgery were retrospectively analyzed, of whom 37 patients（group A） received comprehensive staging surgery under laparoscope and 42 cases （group B） underwent conventional abdominal open surgery. Clinical outcomes were compared between two groups. Results The operations were successful in two groups. The blood loss during operation was less in group A than that in group B（P〈0. 05）. The times of getting off bed,postoperative exhaust and hospital stay were significantly shorter in group A than those in group B（P〈0. 05）. But surgery of group A took a longer time than that of group B（P〈0. 05）. There were no significant differences in the number of lymph nodes resected, the incidence rates of postoperative complications, postoperative staging rising, chemotherapy and relapse during 6-36 months follow-up （P〉0. 05）. Conclusion The outcomes of comprehensive staging surgery under laparoscope for early-stage ovarian cancer are similar to those of conventional abdominal open surgery. But the former has the advantages of less injury and quiker recovery and can be used as a new technique for early-stage ovarian cancer.
