下咽癌侵犯范围及其术前分期的CT评价

摘　　要：目的　探讨CT扫描对下咽癌侵犯范围及其术前分期的价值。方法　回顾性分析 19例下咽癌的CT表现 ,根据CT征象判断肿瘤的侵犯范围及其术前分期 ,并将此结果与手术病理结果相对照。结果　CT判断肿瘤侵犯邻近部位或结构的敏感性为 95 0 % (113/ 119) ,阳性预测值为 85 0 %(113/ 133) ,假阴性率为 5 0 % (6 / 119)。术前CT对原发肿瘤分期的准确性为 79 0 % (15 / 19)。结论CT能较准确地反映下咽癌的侵犯范围及其术前分期 ,这对于下咽癌的临床处理有很大的帮助。Objective To evaluate CT in assessing invasive range and preoperative staging of the hypopharyngeal carcinoma. Methods CT information of 19 cases with hypopharyngeal carcinoma was analyzed retrospectively. The tumor invasive range and staging with CT was correlated with operation-pathologic results. Results The sensitivity and positive predictive value of CT in assessing invasion of the hypopharyngeal carcinoma was 95.0% (113/119) and 85.0% (113/133)respectively, and 5%(6/119) invasion was missed. The accuracy of tumor staging was 79.0% (15/19). Conclusion CT is useful in assessing invasive range and preoperative staging of the hypopharyngeal carcinoma, and helpful to clinical treatment.
