摘 要:目的 探讨HCC中p2 1WAF1与p5 3的表达及其意义。方法 应用免疫组化法检测 38例手术切取的HCC及其配对癌旁肝组织、5例正常肝组织中p2 1WAF1和p5 3的表达 ,并分析它们与病理形态的关系。结果 p2 1WAF1、p5 3在HCC中的表达明显高于癌旁肝组织 ,p2 1WAF1阳性 14例 (36 8% ) ,p5 3阳性 2 8例 (73 7% ) ,其中两者表达一致 (均阳或均阴 )的 18例 ,表达不一致的 2 0例。癌旁肝组织 1例 (2 6% )、正常肝组织 2例p2 1WAF1呈阳性表达 ,所有癌旁及正常肝组织中均未见p5 3表达。HCC中p2 1WAF1表达与p5 3表达无明显关系 (P >0 0 5 )。在不同组织学分级、有无肝内转移或门脉癌栓之间p2 1WAF1、p5 3表达差异均无显著性 (P >0 0 5 )。结论 p2 1WAF1与p5 3在癌内表达均高于癌旁 ,但两者间并无相关性 ,提示p2 1WAF1表达可能受p5 3依赖和p5Objective To explore the expression and significance of p21 WAF1 and p53 in HCC. Methods Immunohistochemical method (IHC) was used to localize and semi quantitate the proteins of p21 WAF1 and p53 and to observe the relationship between the expression of p21 WAF1 and the different histopathologic characters in 38 patients of HCC and their peri cancer tissue as well as 5 normal liver tissue. Results Of all 38 cases, both p21 WAF1 and p53 expression were significantly higher in tumor than that in corresponding non tumors liver tissue; 14 (36 8 %) of 38 cases showed p21 WAF1 positive staining, 28 cases (73 7 %) were p53 positive, p21 WAF1 +/p53+ or p21 WAF1 /p53- were observed in 18, while 20 cases showed p53+/p21 WAF1 - or p53-/p21 WAF1 +. p21 WAF1 + was seen in 1 of 38 (2 6 %) corresponding non cancerous tissue and 2 of 5 normal liver tissue. p53 protein was not detected neither in the non tumorous tissue nor in normal liver. No significant association was found between the expressions of p21 WAF1 and p53( P >0 05) in HCC. Their was no significant correlation between p21 WAF1 or p53 expression and the different histopathologic characters of tumor (differentiating grades, intrahepatic metastasis and/or cancerous thrombi within portal veins). Conclusion Both p21 WAF1 and p53 proteins are over expressed in HCC than that in corresponding non tumorous liver tissue, but there is no relationship between them. Both p53 independent and p53 dependent mechanism may play a role in regulating p21 WAF1 expression in HCC. p21 WAF1 immunostaining cannot be used to assess the status of p53 in any given cell or tissue.
