胰岛素抵抗与ATCI及血浆脂质含量的关系探讨

摘　　要：为探讨胰岛素抵抗 (IR)在动脉粥样硬化性血栓性脑梗死 (ATCI)发病中的作用以及 IR与血浆脂质含量的关系 ,对 5 6例 ATCI患者及 30例正常人的空腹血糖 (FPG)、空腹血清胰岛素 (FINS)及血浆脂质成分进行了测定 ,并计算胰岛素敏感指数 (ISI)。结果显示 ,ATCI患者 FINS显著升高 (P<0 .0 0 1) ,ISI显著降低 (P<0 .0 0 1) ;ISI与高密度脂蛋白胆固醇 (HDL- C)、载脂蛋白 A1 (apo A1 )呈显著正相关 (r=0 .42 ,0 .34 ;P<0 .0 1,0 .0 5 ) ,而与甘油三酯 (TG)、低密度脂蛋白胆固醇 (L DL- C)呈显著负相关 (r=- 0 .39,- 0 .2 8;P<0 .0 1,0 .0 5 )。提示 ATCI患者存在 IR,其To investigate the relationship between insulin resistance (IR)and atherothrombotic cerebral infarction(ATCI)and contents of plasma lipidFasting plasma glucose(FPG),fasting insulin(FINS)and lipids were determined,and insulin sensitivity index(ISI)were calculated in 56 cases of patients with ATCI,only nondiabetic subjects with normotention were chosen to participate in this studyThe correlations between ISI and contents of plasma lipid were also analysedResults showed that FINS level was significantly elevated (P<0001)and ISI was markedly decreased(P<0001)in patients compared with the control(n=30)Moreover,ISI correlated positively with HDL-c (r=042,P<001)and apoA1(r=034,P<005),while correlated negatively with TG(r=-039,P<001),LDL-c(r=-028,P<005)The results suggest that IR in association with dyslipidemia may play a crucial role in the development of patients with ATCI
